The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders. Documents and updates relevant care and procedures information to ensure patient safety. Assists clients with activities of daily living such as serving meals and feeding, client transports, bathing, dressing, skin care, toileting needs; oversees client exercise routines, as applicable. Participates in quality improvement and program activities to maintain consistent level of care and to ensure that interdisciplinary team members work collaboratively. Maintains a safe working environment for the patient and others by adhering to established safety policies; reports unsafe conditions and/or malfunctioning equipment. Assists in the orientation and education of new staff, students and other members of the interdisciplinary team, in collaboration with the Manager or designate. Performs other related duties as assigned. Qualifications Education and Experience Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current First Aid, CPR, and Food Safe certificates. Completion of or eligible to complete MHSU Competency Education. Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage. Skills and Abilities Ability to communicate effectively, both verbally and in writing. Physical ability to carry out the duties of the position. Ability to work independently and in cooperation with others. Ability to operate related equipment. Ability to organize and prioritize. Ability to establish and maintain rapport with patients.
