Job description

Position Overview

The Nurse Practitioner (NP) provides direct, specialized, comprehensive nursing assessments, case management, lifestyle counseling, and education emphasizing on health promotion, and disease prevention strategies to positively affect health outcomes. The NP works in an extended nursing role demonstrating a high level of autonomy and expert skill in order to formulate clinical decisions, appropriately manage acute illness and promote wellness within their area of specialty.

The NP explicitly consults the inter-professional team where care is required outside of the NP scope of practice and/or not covered by a Medical Directive. The incumbent acts as a liaison and consultant to other health care providers and regional partners regarding patient care and health services within the LHIN. She/he provides leadership in the transfer of knowledge into practice in nursing and related fields appropriate to the clinical focus.

What we are looking for

REQUIREMENTS:

- Master Degree – Registered Nurse in the Extended Class including Narcotics prescribing capability. (Nurse Practitioner)

- Certificate of registration from the College of Nurses of Ontario

- Minimum of 3 years related clinical experience as a RN or Nurse Practitioner in a hospital or community setting.

- Experience with adult and geriatric population and experience working with inter-professional teams including physicians.

- Strong understanding of utilization management with a focus on transitioning patient to the most appropriate setting from ‘hospital to home’.

- Excellent understanding of emergency department ambulatory services (EDAS).

- Excellent organizational and interpersonal skills

- Strong leadership skills

- Knowledge of community resources.
- Strong presentation/public speaking skills

- Possess a working knowledge of both official languages.

- Must have a good attendance record.

- Must have positive attitude and compliant to HGH’s values

- Satisfactory review of the Human resources file


HOURS OF WORK:

Temporary Full-time- Maternity leave replacement until September 2027

Rotations: Day, Evening and weekend, possibility of on-call duty when on the unit
