1. Role Summary :
The Registered Nurse / Registered Psychiatric Nurse (RN / RPN) is a DC 3 Level 4 nursing role which enhances the quality of life of individuals by working collaboratively and in partnership with the interdisciplinary team in the provision of person-centered care. As such, this position provides collaborative leadership alongside the Director of Licensed Care Operations through delegation of task and clinical supervision of the licensed care team. This role also provides direct patient care including promoting resident choices, individualized care plans, encouraging resident and family involvement in care decisions and supporting continuity and consistency of care. Utilizing a Psychosocial Rehabilitation outcome-oriented approach and reflecting evidence-informed practice, the RN / RPN provides direct nursing care designed to meet the needs of residents and their families. Works in partnership with other members of the interdisciplinary team by sharing information, demonstrating and providing guidance and delegation of clinical care. This position reports to the Director of Licensed Care Operations.
Duties and Responsibilities :
- Works collaboratively and in partnership with leadership team taking direction from the Director of Licensed Care Operations.
- Collaborative clinical support, supervision and leadership of all aspects of clinical operations.
- Works collaboratively and in partnership with the interdisciplinary team to identify current potential care concerns and strengths, plans interventions; documents and performs planned interventions and evaluates outcomes, as required.
- Provides person-centered, direct nursing care and develops care priorities and clinical profiles in accordance with established policies, procedures, Licensing requirements and standards of nursing practice as set out by the British Columbia College of Nurses and Midwives (BCCNM). Modifies care as needed and communicates changes to interdisciplinary team members in a timely and appropriate manner.
- Maintains resident records by collecting and documenting information from a variety of sources using skills of observation, communication and physical assessment to enable the provision of quality care.
- Documents relevant observations, assessments, interventions and communications, including resident and family teaching and evaluation of care using both the physical and electronic health record (“EHR” is the accepted universal term for any and all legal documentation platforms. ICMHA uses hybrid of both physical and electronic at our Licensed Care sites);
- Works in partnership with the multi-disciplinary team by collegially sharing information, demonstrating and / or providing guidance on clinical tasks, consulting with staff, and providing verbal input into staff performance, when requested.
- Delegate nursing tasks to Licensed Practical Nurses (LPNs) and unlicensed care providers (“UCPs” is the universal term used for our MHWs) ensuring they are performed safely and effectively and intervening as necessary to ensure quality resident care.
- Monitor and evaluate the performance of UCPs engaging in delegated nursing tasks and provide ongoing support and training;
- Develops and oversees the implementation of individualized care plans for residents; modifies care plans as required based on changing resident care needs and priorities;
- Delegation of Task oversight, related teaching and process improvement and provides clinical insight to incident reports and investigations when necessary.
- Communicates resident assessments and changes to Director of Licensed Care Operations, Psychiatrists, and the interdisciplinary team;
- Participates with the Pharmacist in regular medication reviews and the medication safety and advisory council (MSAC);
- Participates in quality improvement and risk management activities such as participating in safety audits and identifying needs for corrective action / improvements, gathering information and providing input into proposed changes to identified clinical practices, procedures, or protocols;
- Liaises with relevant Island Health programs and community agencies in coordinating and integrating mental health services;
- Participates in the orientation and continuing education of medication management for licensed care staff;
- Provide health education to residents and their families on disease management, treatment plans, and preventative care;
- Maintains a safe secure environment for residents and staff by providing information and support through health and safety guidelines, ensuring equipment is in safe working condition, reporting unsafe, hazardous conditions and suspected resident abuse. Takes immediate action, as appropriate, to reduce / eliminate recognized safety risks and documents the incident / unusual occurrence following established procedures. Complies with WorkSafe BC requirements including completing safety audits as assigned.
- Lead, mentor, and oversee the practice of nursing staff (LPNs), and report any practice concerns to the DLCO and HR;
- In collaboration with the ED and DLCO, develop, revise, and implement policies and procedures to ensure high standards of care;
- Facilitate interdisciplinary team meetings to discuss and plan resident care as well as provide education and updates related to clinical practice;
- Communicate with resident families as needs arise and participates in resident and / or family meetings;
- Facilitate routine resident care conferences to enhance the resident experience;
- Ensure comprehensive and coordinated care delivery to all residents at ICMH.
- Collect and analyze data to resident outcomes, staff performance, and resource utilization; Prepare and present reports on data to senior management, with recommendations for quality improvement.
- Engage in ongoing professional development and maintain knowledge of current advancements in nursing practice and mental health care;
- Advocate for resident rights and access to high quality mental health services.
- Ensures the MAR / EMAR and Narcotics Record are aligned in accordance with provincial regulations.
- Administers medication to residents and provides medication reminders, in accordance with regulatory body (BCCNM) requirements and organizational policy and procedures;
- Safe administration of medication, including injections, and conducts follow up assessment, intervention, and documentation as needed;
- Submit cash requests for weekly distribution of resident funds, including fund distribution to residents, in collaboration with the Finance department;
- Participates in the Nurse-on-Call Rotation;
- Performs other related duties as assigned
