Job Responsibilities
Provide skilled physical therapy services to patients in home health settings, developing and implementing individualized treatment plans to improve mobility and manage pain.
Assess patient needs and progress regularly, adjusting therapy interventions accordingly to ensure optimal outcomes.
Document patient care accurately in the EMR system, ensuring compliance with facility and regulatory standards.
