Job Requirements

Education Requirements : Valid Physical Therapist license in the State of California

Certifications Requirements : CPR certification, Basic Life Support (BLS), and State licensure

Job Responsibilities

Provide skilled physical therapy services to patients in home health settings, developing and implementing individualized treatment plans to improve mobility and manage pain.

Assess patient needs and progress regularly, adjusting therapy interventions accordingly to ensure optimal outcomes.

Document patient care accurately in the EMR system, ensuring compliance with facility and regulatory standards.

Collaborate with family members, caregivers, and healthcare team members to coordinate care and improve patient support.
