Job Requirements

Education Requirements : Bachelor’s degree in Physical Therapy or a related field from an accredited institution.

Certifications Requirements : Current and valid state Physical Therapist license; CPR certification; Malpractice insurance coverage.

Experience Requirements : At least 1 year of home health physical therapy experience; Experience working with Electronic Medical Records (EMR) systems; Proven ability to manage 5-6 visits per day efficiently.

Other Requirements : Ability to work daytime hours from 8 AM to 5 PM; Valid driver’s license and reliable transportation within a 30-mile radius; Strong communication and interpersonal skills; Ability to work independently and follow treatment plans closely.

Job Responsibilities

Provide comprehensive physical therapy assessments and develop individualized treatment plans for home health patients within the designated radius.

Administer therapeutic interventions and exercises to improve patient mobility, strength, and pain management.

Document patient progress, treatment plans, and clinical notes accurately in the EMR system (Matrix).

Collaborate with patients, families, and other healthcare providers to ensure coordinated care and optimal outcomes.

Manage a productivity schedule of 5-6 patient visits per day, ensuring timely and efficient service delivery.
