about the role :

Sherbourne Health has been funding by Immigration, Refugee and Citizenship Canada (IRCC) to provider our Finding Health Project.

Our project is dedicated to delivering services at the intersection of health and settlement. We are seeking a committed Case Manager to join our team and assist newcomers in navigating the healthcare and settlement systems in Canada. This role offers a unique opportunity to make a significant impact on the lives of newcomers by facilitating their integration into Canadian society. The Case Manager will work at Sherbourne Health and other locations, including the community hub at St. James Town Community Corner.

Responsibilities and Tasks :

  • Conduct comprehensive assessments of newcomer clients seeking case management support, including Needs and Assets Assessment and Referral Service (NAARS) evaluations, to identify individual needs and goals.
  • Confirm eligibility for services based on the IRCC Case Management Framework
  • Develop personalized settlement plans in collaboration with clients, outlining actionable steps towards achieving their objectives.
  • Provide referrals to a wide range of essential services, including settlement agencies, healthcare providers, social assistance programs, employment resources, and community organizations, including Francophone settlement organizations.
  • Assist clients in completing Health Equity Data Questionnaires to ensure equitable service provision.
  • Collaborate with inter-professional care teams to gain a holistic understanding of clients’ healthcare requirements and develop appropriate support strategies.
  • Offer comprehensive client navigation and service coordination, guiding newcomers with high needs through complex systems and processes.
  • Provide accompaniment services to clients facing barriers to independent navigation, ensuring access to critical appointments and resources.
  • Empower clients to adapt to life in Canada by utilizing a strengths-based approach, building on their existing skills and resilience.
  • Monitor the success of referrals and evaluate their effectiveness in meeting client goals, adjusting as needed.
  • Conduct exit surveys to gather feedback and optimize Case Manager service delivery and capacity.
  • Advocate on behalf of service users, with their consent, to ensure access to services and promote their rights.
  • Maintain up-to-date knowledge of available community resources and share this information with service users.
  • Ensure accurate and timely documentation of service user data in relevant health and settlement databases, as well as Sherbourne Health’s Electronic Medical Record (EMR) system.
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, clients, families, etc.) by taking all reasonable precautions and complying with Sherbourne-related policies, health and safety legislation and best practices.
  • Participate in the committees, team meetings, and Sherbourne working groups as requested.
  • Participate in continuous quality improvement initiatives.
  • Other activities as assigned.
  • To thrive in this role, you’ll need

    Education / experience / knowledge and skills :

  • An undergraduate degree or diploma in the social service, social work, social sciences and at least three years of experience in the settlement services sector or newcomer health sector or an equivalent blend of education and transferable experience.
  • Professional experience working with diverse newcomer and refugee populations, including LGBTQ newcomers, racialized newcomers and / or newcomers experiencing homelessness or precarious housing.
  • Demonstrated understanding of the importance of health and mental health in the newcomer settlement experience.
  • Excellent organizational and time management skills and ability to work and deliver within tight timelines.
  • Experience working in an interdisciplinary team.
  • Excellent report writing and presentation skills.
  • Familiarity with client databases.
  • Strong PC / Software skills.
  • Fluency in a second language is an asset.

    The regular hours of this position are Monday to Friday, from 9 : 00 am – 5 : 00 pm; however, this position will necessitate occasional work on evenings and weekends to be available when newcomers and services are available. In addition, this position requires working in a fast-paced environment meeting the needs of multiple stakeholders. Therefore, time management, multi-tasking, initiative, organization and prioritizing are essential.
