Come join the Calgary Zone rural social work team at Oilfields Hospital in Black Diamond! We are a strong, supportive team committed to creating a positive work environment with various opportunities for growth and development. Reporting to and working alongside the Acute Manager and Unit Manager, Oilfield’s Hospital social worker functions as a key point of contact to assist patients and families in the Emergency Department, Medical Inpatient Unit and Long Term Care Facility. Using AHS core values and a patient family centered care model the social worker for Oilfields Hospital will lead transition service support, complex discharge planning and proactive client management. This position works with an inter-disciplinary team and the successful candidate must have excellent verbal and written communication skills, adaptive personality, along with the ability to think critically.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Required Qualifications:Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications: Preferred Qualifications:Creative, innovative, solutions focused to manage complexity. Team player who can also work independently. Experience with program development. Experience working in a rural setting.
