Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients / clients / residents and families : As a member of the CQPS unit based quality improvement team, the Senior Coordinator, Quality Improvement (QI) supports clinical leaders and point-of care staff by providing coordination, education and guidance in the application of QI principles, evidence-based practices, approaches and tools, in order to facilitate QI knowledge translation, sustainable change and enable appropriate harm prevention strategies.
Collaborates with Systems Optimization, Infection Prevention & Control, QI Audit, Accreditation, PSLS and Quality Review Team and others to collate relevant data.
Works with a portfolio of assigned acute and / or LTC units which have implemented unit-based QI within the designated geographical areas of Fraser North, Fraser South and / or Fraser East.
Provides QI leadership, mentorship and daily support to unit based QI teams and their leadership. Contributes to the development of a culture of excellence within QI and unit-based QI teams.
Responsibilities Initiates site preparation meetings with Directors, Managers and Medical Staff of the site, by liaising with the site regarding implementation requirements, specific education needs, and orientation schedules.
Facilitates site planning meetings with operational leaders to share knowledge of unit-based QI objectives and facilitate understanding of unit-based QI, confirms engagement by leadership, identification of staff champions, and planning of dates for orientation and start date(s).
Leads unit planning meetings with designated unit-based QI teams, eg, operational leaders, medical staff quality lead, unit champions, Educators and PCCs, by setting agenda in consultation with site, working with operational leadership to determine next steps for unit-based QI implementation, and providing ongoing consultation and sharing of knowledge regarding unit-based QI opportunities and benefits.
Responsible for ensuring completion of unit and site based unit-based QI reports. Presents reports for unit-based QI units following approved formats.
Provides education and / or advisement for unit-based QI team on value of the data in identifying strengths and opportunities for improvement.
Provides direction and support to unit-based QI teams regarding collection of available data to identify current standing of unit in relation to Accreditation Canada standards, reported patient safety event themes, infection reports, PSP results and other quality indicators.
Designs, develops and delivers unit-based QI education, including methodology, best practice, quality improvement tools, and applicable change theory.
Provides education specific to site requirements and considers employees, FH medical staff and patient representatives. Supports site / unit staff to facilitate unit-based QI training workshops.
Analyzes overall unit-based QI program requirements to develop templates for training and communications in support of project-related and operational requirements.
Develops and prepares unit-based QI materials for presentations, workshops, and new employee / manager orientation. Develops self-support education materials such as quick references, FAQs and other instructional material to assist FH staff.
Oversees development of, and signs off on, education webpages and related unit-based QI content, SharePoint sites and shared drives to ensure information is current and easily accessible by appropriate stakeholders.
Provides ongoing support and direction to unit-based QI teams, regarding areas such as data collection and management of the QI process, as required.
Provides on-site support to unit-based QI teams by being a first point of contact for support and issues that may arise such as change management strategies for QI, confirming unit initiatives align with FH strategic priorities, strategies for engagement of unit staff, and other related troubleshooting.
Escalates issues to unit-based QI manager and CQPS manager as required. Develops and maintains relationships with internal stakeholders in relation to coordination and implementation of QI practices, including Infection Control, Professional Practice, FH network and programs, Site Leadership, Clinical Nurse Specialists, Clinical Nurse Educators, Allied Health practitioners and frontline nurses, National Surgical Quality Improvement Program staff, and PSLS staff.
Provides timely and consistent information to Site Leadership and physicians as appropriate, Provides orientation, mentorship and guidance to new CQPS associates, including sharing of updated QI methodologies, current information, education materials and resources.
Maintains professional development through keeping abreast of current practices in quality improvement model(s), data collection, data display and interpretation and applicable change management.
Collaborates with CQPS and unit-based QI team and Leadership by proactively sharing information. Participates in internal and external meetings and committees as assigned, representing unit-based QI at site quality leadership meetings such as Hospital Quality Operations and Management Committee (HQOMC), site quality subcommittee, program network meetings, and engaging in conversations to inform improvement activities and identify opportunities from a site based, geographical or regional lens.
Qualifications Education and Experience Degree in Health, Social Sciences or related field, plus three years related experience in a healthcare setting including knowledge / courses in Adult Education, Change management, and Quality Improvement, or an equivalent combination of education, training and experience.
Valid BC Driver's License and access to a personal vehicle for business purposes. Competencies Demonstrates the leadership practices of the Fraser Health Leadership Framework of Clear, Caring and Courageous and creates the conditions for people to succeed.
Professional / Technical Capabilities : Broad knowledge of Canadian healthcare delivery practices and knowledge and / or education of QI principles and methodologies.
Knowledge of project management principles, methods and tools with the ability to plan, manage and complete projects. Ability to work independently and in an integrated multi professional team environment.
Knowledge and ability to apply applicable change management methodologies. Demonstrated instructional program design and delivery skills.
Demonstrated ability to lead project teams. Demonstrated ability model a variety of teaching methods. Demonstrated clinical thinking and analytical skills including analysis and evaluation methodology.
Proficient in MS Office, including an ability to use Excel to create Quality charts and use Word / Excel or Visio to document basic flowcharting functions;
Ability to utilize multiple electronic database systems and applicable software applications. Ability to facilitate education and training.
Ability to provide work direction to team members. Excellent interpersonal, problem-solving and conflict resolution skills.
