Jobresponsibilities :
- Workingwith the team lead they will develop appropriate databasescontribute to reports as necessary report any issues / concerns thatmay arise and make sure that the service meets all targets in theservice specification.
- Works in partnershipwith the local Health Visiting service to ensure seamless care asthe child progresses from the early years intoschool.
- Adheres to and works within local andnational child safeguarding policies in the prevention and earlydetection of abuse in all age groups.
- Developsaccurate factual reports and attends case conferences and othermultiprofessional meetings as required for identified children andtheir families.
- Meets with all identifiedchildren at least once a term and refers / signposts to otherprofessionals / services if identified.
- Works inpartnership with primary care ensuring that where appropriate therelevant GP practice is notified of CYP health concerns to securean integrated system of care that safeguardsthem.
- Works in partnership with other keystakeholders to promote the welfare and safety ofCYP.
- Offers strategic support to schools inorder to contribute to reducing the number of children who enterthe safeguarding system through preventativework.
- Works in liaison with identified schoolstaff to contribute to the delivery of health improvement promotionand preventative approaches through signposting to other healthcareprofessionals / services and available resources and support staffwith appropriate training.
- Promotes emotionalwellbeing through the schoolaged years working alongside childrenand young people to support those with emotional and mental healthdifficulties and referring to CAMHS whereappropriate.
- Has knowledge of other availableresources to support emotional and mental wellbeing e.g. onlinetraining packages.
- Work in partnership withprimary and secondary care colleagues to support children and youngpeople with longterm conditions or complex needs signposting andreferring school staff to appropriate support and facilitatingcorrect management of health conditions to ensure hospitaladmissions are kept to a minimum.
- Works withthe senior management team to develop a comprehensive Action Planfor the school which includes all policies procedures andguidelines as an overview of SEN safeguarding and pastoral andmedical needs together.
- Attends regular teammeetings to promote effective communication and team leads byexample promoting best practices and high standards at alltimes.
- Works with the Team Lead to ensure thatall policies procedures and guidelines are monitored and reviewservice delivery to ensure the service meets both national andlocal requirements.
- Manages skill mix staffeffectively addressing issues such as poor performance andinforming the Service lead of these issues in a timelyfashion.
- Participates in the orientation andinduction of new staff promoting good interpersonal relationshipswithin the team.
- Acts as a mentor in theeducation and training of pre and postregistration students.Facilitates other students as required.
Acts as a preceptor fornewly qualified nurses / specialist practitioners in the SchoolNursing Service
- Acts as a resource for skillmix staff offering guidance support andsupervision.
- Identifies own learning needs andthat of skill mix staff within theteam.
- Remains up to date with currentprofessional issues e.g. by attending professional forums readingprofessional journalsetc.
