Purpose :
Toassist with the provision of therapeutic social / recreational activities,Activities of Daily Living (ADLs), medication administration, day and overnightmonitoring and behavior interventions for Adult Day Program and Overnight StayProgram participants (which includes frail elderly, dementia, mental health andacquired brain injury population).
KeyAccountabilities and Success Criteria :
- Documenttreatment protocols and medications administered, unusual incidents, nearmisses, advice given to client or caregiver and any other pertinent informationon client chart
- Assistin maintaining a safe physical and psycho-social environment.
- Assistthe Overnight Stay Team and Clinical Resource Worker with the completion of dayprogram client profiles and care plans
- Assistwith the implementation and organization of client Instrumental Activities ofDaily living; Assist clients with activities of daily living.
- Participatein meal preparation and in recreational activities
- Complywith St. Joseph's Health Centre’s Privacy policy based on PHIPAprinciples.
- Troubleshoot for staff, participants, families, and physicians.
- Promoteand support ongoing staff professional development.
- Providenursing care in accordance with administrative policies, established standardsoutlined in the Standards of Nursing Practice from the College of Nurses ofOntario Provide support andassistance during staff absences.
- Administermedications as prescribed by participant’s physician and requested byparticipant’s family and maintain an accurate record in MAR.
- Assessparticipant’s ability to ambulate and perform lifts and transfers, assistparticipant with ambulation as necessary.
- Provideappropriate interventions with participants exhibiting responsive behaviours,communicate and document outcomes.
- Respondappropriately to emergency situations and provide direction to other teammembers.
Qualificationsand Skills :
- Currentregistration with the Ontario College of Nurses.
- CurrentCPR certificate
- Registrationwith or completion of a Geriatric Certificate Program (preferred through theRegional Geriatric Program)-UFIRST or PIECES Certification
- Advancedknowledge of diseases and conditions causing memory impairment
- Experiencewith community based cognitively impaired, frail elderly population
- Experiencein administering medications and monitoring client response to medications
- Experiencewith recreational program planning and delivery
- Excellentdocumentation, organizational, interpersonal and communication skills.
- Knowledgeof pertinent community support services preferred.
- Abilityand willingness to work as an effective team member.
- Abilityto apply effective body mechanics and meet physical demands of the position.
- Knowledgeof and / or willingness to pursue learning in the InterRAI CHA and PIECESassessment tools
- Completionof Pharmacology certificate preferred
- FoodHandling Certificate preferred
