Detailed Overview :

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients / clients / residents and families :

Under the Pathway to Hope, the Province has established Integrated Child and Youth (ICY) teams in several communities defined by school district boundaries.

Each ICY team supports a cluster of Public, Independent and First Nations operated schools and will include all children and youth within the geographic region, including those who do not attend a school.

These community-based multidisciplinary teams coordinate and deliver wraparound mental health and substance use services and supports for children and youth 0-19 and their families.

Integrated Child and Youth teams are comprised of team members employed by various organizations and ministries.

Reporting to the Director or Manager as assigned, the Lead, Integrated Child and Youth Program is a dynamic multi-functional administrator role that champions integration of service across the team and community, plays a critical role in ensuring fidelity to the ICY model of care, and leads team integration and cohesion to enable effective provision of mental health and substance use supports to children, youth and families in the applicable service area.

The service delivery will be flexible and outbound, reflecting the preferences of children, youth, and families. Through the collective impact of partners working together in an integrated way, children and youth with mild, moderate, and severe mental health and / or substance use challenges and their families / caregivers will have timely, equitable access to a seamless system of culturally safe and effective interventions that are based on the unique strengths, needs and risks of children, youth and families.

Responsibilities : Leadership of Team

Leadership of Team

Provides leadership and manages integration of all ICY multi-disciplinary team members by methods such as coaching, mentoring, training, developing, and providing practice support.

Fosters cooperation, collaboration, communication, and harmonious relationships among all team members.

Manages and evaluates performance to improve individual and organizational performance, taking corrective action as needed in accordance with Fraser Health policies and procedures;

administers, with support from human resources, applicable collective agreement provisions of staff in direct reporting relationships.

  • Provides expert consultative advice to other health care practitioners and acts as a resource for the assigned clinical operation(s) or service contracts.
  • Communicates regularly with employer organizations for on-going collaboration, integration, issues management, and to ensure the mandate of ICY teams is being met.
  • Develops and maintains fidelity of the ICY model of care, and provides guidance and direction on the interpretation of Acts, regulations, standards, policy, and procedures.
  • Delivers onboarding and orientation to all team members. Assesses training needs and supports training opportunities by methods such as delivering training modules and providing access to available education / training through partners and in community for all ICY team members.
  • Supervises assigned Fraser Health staff including responsibility for hiring, firing and disciplinary actions.
  • Represents the Employer in a confidential capacity in matters relating to labour relations or personnel.

Coordination & Monitoring Service Delivery

  • Oversees efficient operations of the service, including equitable workload distribution to ensure that children and youth requesting service receive timely responses from ICY team members.
  • Ensures that practice principles, including building resiliency and innovation, are embedded in ICY team practice, and ensures that team members bring an explicit diversity, inclusion and equity lens to service design and delivery.

Ensuring a strengths-based, child / family-centred, culturally safe, and trauma informed approach to care is employed by all team members.

  • Provides oversight to the establishment of the integrated care plan by such methods as facilitating check ins, team meetings, case reviews and overseeing closing reports with all ICY team members, and ensures child / youth and family care plans are appropriate to their needs.
  • Models and ensures that ICY Team staff provide integrated care while demonstrating confidentiality regarding children / youth, their families and treatment programs.
  • Oversees collection of data for monitoring and evaluation, and supports team efficacy in use of ERP / clinical information systems.
  • Develops and maintains an effective service through continuous quality improvement activities such as by leading community involvement in evaluation of ICY teams.
  • Manages and is financially accountable for ICY operational budget and procurement.
  • Ensures compliance with complaint review and audit processes.

Coordination with Partner Organizations & Community

  • Establishes and implements program standards of care, protocols, procedures and safety precautions that are consistent with the mission, philosophy and policies of the employer organizations of ICY team members, current legislation and accreditation standards.
  • Oversees and coordinates the day-to-day activities of staff and standards of care; consulting with appropriate clinical leads when necessary.
  • Assesses and arranges admission of children, youth and families to ICY services within the context of a coordinated intake process.
  • Works to ensure an integrated system of care for children, youth and families by methods such as providing oversight and organization of an integrated response around child / youth and family individual cases, through close collaboration with Health Authority leadership, MHSU leaders and programs both internally and externally, and through consultation and collaboration with community partners in mental health and substance use services.
  • Facilitates discussion and consensus building among clinicians and practitioners, bringing in other experts and perspectives as needed.
  • Supports the commitment to embed cultural safety and humility into practice through ongoing collaboration with Indigenous community partners, and internal / external stakeholders including leadership.
  • Ensures optimal utilization of mental health and substance use resources by establishing strong partnership linkages, maintaining regular meetings both within ICY teams and in community, and ensuring that necessary reporting mechanisms and systems are in place.
  • Participates in local committees, internal / external case conferences and planning bodies by methods such as leading meetings and / or providing input, facilitating resolution of issues, and advocating the goals and objectives of mental health and substance use services.
  • Advocates for and promotes public awareness of ICY teams through supporting the development of a public education strategy to the community.

Qualifications :

Education and Experience

Master's degree in nursing, psychology or social work or other relevant health care field, and five (5) years' recent, related clinical consultation / supervision, administration, community development, project management experience including understanding and knowledge of Indigenous Health issues, protocols and health needs, or equivalent combination of education, training and experience.

Valid Class 5 Driver's license and access to personal vehicle for business related purposes. Applicable professional designations as required.

Current CPR "Level C" Basic Rescuer certificate, as required.

  • Ability to find creative solutions to problems and to develop alternative strategies to support quality improvement initiatives.
  • Ability to assess training needs and facilitate training opportunities such as ICY Core Module training as well as other training available through partners.
  • Ability to establish framework and practices specific to ICY within team and with new team members.
  • Positive problem-solving skills in a dynamic work environment.
  • Strong planning, critical thinking and analytical skills.
  • Strong computer proficiency in client / patient information systems, electronic scheduling systems, and office suite software.
  • Extensive knowledge and ability to direct quality assurance processes, professional development and professional competency issues.
  • Background knowledge in the field of addictions and clinical practice including familiarity with trauma informed practice, motivational enhancement and client-directed, outcome informed methodologies.
