The Role : As a Social Worker in the Primary and Community Care unit, you will provide essential support to individuals and families facing various challenges related to health, mental well-being, and social needs.

You will work collaboratively with an interdisciplinary team to assess client needs, develop intervention plans, and facilitate access to community resources, enhancing overall patient care and outcomes.

Responsibilities :

  • Conduct comprehensive assessments to identify clients' social, emotional, and environmental needs.
  • Develop and implement individualized intervention plans that address clients' unique challenges and goals.
  • Provide counseling and support to clients and their families to help them navigate complex health and social systems.
  • Collaborate with healthcare professionals, including nurses and occupational therapists, to ensure a holistic approach to patient care.
  • Advocate for clients' rights and facilitate access to community resources and services.
  • Maintain accurate and up-to-date client records and documentation, ensuring compliance with relevant policies and regulations.

Requirements :

  • Current registration and license to practice as a Social Worker in the applicable jurisdiction.
  • Minimum of 2 years of experience in social work, preferably in a community or primary care setting.
  • Strong understanding of social work practices, mental health issues, and community resources.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team-oriented environment.
