JOB TITLE : Physiotherapist

REPORTING RELATIONSHIP : Manager, Ambulatory Services
MAJOR RESPONSIBILITIES :
1. To provide assessment, management, rehabilitation and treatment for physical problems caused by injury, disease or disability.
2. To organize, plan and assign the patient caseload.
3. To develop and implement patient care plans which may include exercises, assistive devices, or treatment with manual therapy according to professional scopes of practice as appropriate.
4. Maintain charting standards according to institutional and College standards.
5. To maintain records of workload and service statistics and submit to administration in a timely manner.
6. To participate as a member of an interdisciplinary team in the provision of patient care.
7. To participate in quality assurance programs.
8. To maintain continuing professional development and ongoing learning by attendance at professional seminars, courses and conferences
9. To always maintain patient safety and infection control precautions
10. To provide educational in-services to staff, patients / families and volunteers as needed
11. To establish and maintain liaisons with hospital and community groups through project and committee work as required
12. To provide appropriate referrals and consultation to other professionals and / or agencies as required
13. To ensure and maintain patient equipment is clean and in sound working order
14. To maintain confidentiality of patient information and ensure security of patient records.
15. To provide student placement experiences as caseload and resources permit
16. To attend and participate in Rehabilitation Service staff meetings and other professional practice related activities.
17. To supervise assigned patient care activities of support personnel ie PTA, PTA / OTA, according to College guidelines.
18. To perform other duties as assigned by the Manager, Ambulatory Services

EDUCATIONAL PREPARATION :
- Current registration with the College of Physiotherapists of Ontario
- Eligible for C.P.A., OPA membership.
REQUIRED CERTIFICATIONS :
REQUIRED EXPERIENCE :
- 3-5 years experience within a healthcare setting focused on the adult, senior and rehabilitation populations.
- Basic computer skills including electronic medical record and Microsoft office.
- Effective critical thinking, problem solving, organization and leadership skills.
DESIRABLE EDUCATION / CERTIFICATION(S) :
FIM or willing to obtain within 1 year
PATIENT SAFETY RESPONSIBILITIES :
- Participate in and comply with hospital safety and infection control programs.
- Report unsafe conditions.
- Assist in the reporting and investigation of incidents.
- Review related policies and procedures on a regular basis.
- Maintain patient and hospital confidentiality.
- Respond appropriately to codes.
- Participate in quality activities as required.

Location : South Muskoka Memorial Hospital ( SMMH) located in Bracebridge
Department : Rehabilitation Services
Hours of Work : Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements per the collective agreement.
