Salary :

The Kee Tas Kee Now Tribal Council is comprised of the five member Nations and is responsible for providing guidance and support to the individual member First Nations in developing and achieving success in the management and administration of their own programs with the intent of developing self-reliance.

The Kee Tas Kee Now Tribal Council is comprised of the member Nations of; Loon River First Nation #476, Lubicon Lake Band #453, Peerless Trout First Nation #478, Whitefish Lake First Nation #459 and Woodland Cree First Nation #474.

It was formed in 1995 to facilitate joint action by the member Nations on matters of mutual concern.

Job Description :

As a Nîsohtîtan support worker with KTC, you will act as a support in response to any children / families that have been referred for services.

The Nîsohtîtan support worker duties will include ongoing monitoring of client needs, scheduling of appointments, collaboration in service plan development, building relationship with families, connecting family with service providers, maintaining accurate records, working with external organizations.

The Nîsohtîtan support worker ensures flexible and efficient service to children by working as part of the KTC Health Team.

Responsibilities :

Provide guidance on the appropriate course of action for referrals :

  • Assess referrals
  • Guide the Administrative Support and Referral Coordinator on next steps to for complex or incomplete referrals
  • Follow-up with clients and service providers to ensure referrals are being completed and the family is supported in accessing these services

Service delivery for families :

  • Support service planning by regularly engaging with clients and service providers
  • Act as point of contact for clinicians, external organizations (where appropriate) and families
  • Assess and support family social needs including referrals to external services as required.
  • Build and maintain relationship with community-based service provides, clients and families
  • In-person support to help families implement activities (program plans) recommended by service providers (e.g. : completion of consent forms)
  • Coordinate, complete and submit individual Jordan’s Principle requests
  • Meet with the clinical team, and / or lead as required to coordinate complex needs cases

Support and improve the KTC Children’s Health referral process’ :

  • Promote increased communication among KTC staff to facilitate meeting children’s needs
  • Maintain ongoing tracking and appropriate documentation in client files to promote team awareness.

Skills and Abilities :

  • Effective organizational, written, communication and computer skills are an asset
  • Ability to handle a variety of issues at the same time and work under pressure
  • Problem solve potential issues related to clients accessing services (e.g., request interpreters as appropriate, transportation services or Non-Insured Health Benefit assistance)
  • Ability to work independently

Qualifications :

  • Awareness of the First Nations Health Consortium and Jordan’s Principle is an asset
  • Knowledge of the challenges and opportunities in First Nations communities, including the unique features of remote communities
  • Minimum High School diploma with experience working with and supporting families or experience working in health care is required.
  • Experience working in First Nation organizations / communities and the ability to provide service from a trauma informed and Indigenous perspective.
  • Valid Drivers License

Conditions of Employment :

RCMP / Police Criminal Record Check with Vulnerable Sector and Child Welfare Intervention Record Check (formerly Child Welfare Check, CWIS) submitted prior to commencement of employment.
