Your Opportunity:

GeneScreen is a new initiative that provides comprehensive system navigation support for individuals diagnosed with a hereditary cancer syndrome (HCS). Working closely with geneticists and genetic counsellors at the hereditary cancer genetics clinic, the nurse practitioner (NP) within GeneScreen coordinates longitudinal care for patients diagnosed with an HCS. Patients with these genetic conditions are at risk for multiple different cancer types and require complex care coordination and follow-up through multiple specialties. This position requires specialized knowledge of hereditary cancer, cancer screening guidelines, and management options. Experience with patient navigation within cancer screening and/or treatment would be an asset. The NP in this role will be responsible for reviewing care options with patients, arranging appropriate referrals according to evidence-informed guidelines, reviewing results, and determining any required follow-up care or additional referrals. In addition, they will provide education to patients and their primary care providers about recommended screening intervals and modalities and determine when a patient is appropriate to be discharged back to primary care. The NP will play a key role in designing clinic workflows, developing screening protocols, and implementing evidence-informed provincial screening guidelines for patients with identified genetic cancer risks. The NP will also be involved in ongoing optimization, research, and evaluation of the GeneScreen program. This role is ideal for an individual who is passionate about innovation, genetics-informed care, and improving early cancer detection through structured screening programs.
