This position reports to the Home Care Manager, in Fort McMurray, Alberta. This position provides Home Care services located within the Regional Municipality of Wood Buffalo and outlying communities.
The Home Care Registered Nurse Case Manager provides case management services and interventions to clients with a variety of healthcare needs.
As a case manager, you will be responsible for the oversite and coordination of the healthcare needs of clients across the lifespan, including chronic disease management, health assessments, assessment for assistance with ADL’s, wound assessments, wound care, assessments for placement in DLO facilities as well as palliative and end of life care.

The Home Care Registered Nurse Case Manager is responsible for the assessment, planning, implementation, and evaluation of client care, in the setting most appropriate for the client.
The incumbent performs a variety of nursing duties in accordance with clinical policies and nursing care standards. Note : This position requires a vehicle for business use.
Possession of a valid driver’s license and access to a motor vehicle in good repair for work-related travel is required, and the ability to drive in long distances in all weather conditions.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being.
In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families.
You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance.
You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE : Provisional Permit Holders shall be paid $ - $ Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS / UNA Article or (ii) as applicable.

Required Qualifications :
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required : CRNA Medication Management - Chapters 1 - 4 ( and the NextGenU Health Assessment Resources Certification (

A Master’s Degree in nursing or a related field is preferred. Aids to daily living (AADL) authorization experience preferred.
1-3 years of Home Care Case Management experience.
