Do you have a passion for nursing? We have the position for you! We are seeking a motivated Registered Nurse with a dedication for delivering compassionate patient care to join our team!

In accordance with established vision and values of the organization, the Transition Nurse practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model.

Under the direction of the Manager (or designate), the Transition Nurse - working collegially and collaboratively with multidisciplinary / multi-agency teams - takes a lead role in planning, implementing, evaluating and coordinating the flow of integrated care plans for all assigned clients with complex discharge planning and service needs.

This position provides comprehensive assessment of individuals and is responsible for identifying variances from discharge plans and service needs, as well as resolution of client, caregiver and system issues.

The Transition Nurse acts as a resource to clients / families and various health service teams in the assessment of client needs and eligibility for appropriate resources.

This position is responsible for communicating changes in the day to day service needs of the client and liaising with the multidisciplinary team so that changes in capacity and utilization can be anticipated, monitored and alternate strategies developed in order to facilitate the most effective use of health service resources to meet client needs.

Shift Rotation / Hours of work : Monday to Friday, Days - 08 : 30 to 16 : 30

Fort St. John is known as "The Energetic City," which reflects the large resource base of oil, natural gas, forestry and agriculture.

Overlooking the Peace River, the town of Fort St John is located above the Peace River Valley and serves as the capital of the Peace River district.

The population of Fort St. John is over 21,400 but it services an area of approximately 70,000. The community is the hub of the Northeast with a cultural centre and library, theatre, and art space.

There are seven elementary schools, two middle schools, and one secondary school. There is also a Northern Lights College campus.

Qualfications

  • Registration with BC College of Nurses and Midwives as a practicing RN registrant.
  • Two (2) years recent related clinical experience which includes a combination of Community and Acute Care nursing experience working with clinically complex clients;

or an equivalent combination of training and experience.

Current valid B.C. Drivers' License and access to reliable personal vehicle required.

Skills and Abilities :

  • Assessment and Intervention - Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through appropriate / prescribed technical, therapeutic, safety type interventions.
  • Communication - Demonstrated ability to communicate effectively with the clients, families, the public, medical staff and the members of the interdisciplinary team using verbal, written and computer communication means.
  • Critical Thinking - Demonstrated ability to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively.
  • Human Caring and Relationship Centered Practice - Ability to promote client-focused care that demonstrates care for and with clients and significant others, sensitive to diverse cultures and preferences, client advocacy and social justice concerns.
  • Teaching - Ability to transmit information intended to instruct clients and others about topics essential to health care and well-being.
  • Management - Manages time and resources, implementing activities to promote cooperation among relevant others, collaboration across disciplines and related activities.
  • Leadership - Promotes staff morale, cooperation, assertiveness and risk-taking, creative planning for change and innovations, implementation of NH policies or other protocols, and ongoing professional development of self.
  • Knowledge Integration - Using factual information, prior learning and basic principles and procedures to support decisions and actions with relevant research-based evidence.

Integrates best practice from nursing and health-related disciplines and the humanities, arts and sciences disciplines into professional practice.

  • Ability to operate related equipment including relevant computer applications.
  • Physical ability to perform the duties of the position.
