Job Summary
The Manager of Access and Flow, Bed Access, Bed Allocation, and Patient Transitions is responsible for the overall coordination and optimization of patient flow, bed access, and bed allocation within Southlake Regional Health Centre.
This role ensures the efficient and effective movement of patients through the hospital system, reduces bottlenecks, improves bed utilization, and enhances patient care and satisfaction through strategic planning and real-time decision-making.
Primary Responsibilities
- Develop and implement strategies to optimize patient flow across all hospital departments. This includes monitoring patient movement, identifying bottlenecks, and ensuring timely admissions, transfers, and discharges.
- Regularly analyze patient flow data to identify trends and areas for improvement.
- Oversee the centralized prioritization and placement of patients based on bed availability, patient acuity, and staffing resources.
Make immediate decisions regarding patient placement to maximize bed utilization and minimize wait times.
Facilitate the coordination of patient transitions between levels of care, departments, and external facilities. Ensure continuity of care and reduce the length of stay by managing the discharge planning process and coordinating with external healthcare providers.
Lead Joint Discharge Rounds and Long Stay Meeting
- Lead performance improvement initiatives related to patient flow, bed management, and transitions. Utilize data analysis and evidence-based practices to identify areas for improvement and implement changes that enhance efficiency and patient outcomes
- Collaborate with clinical leaders, department managers, and external partners to develop and maintain effective communication and policies for patient flow and transitions.
Ensure that all stakeholders are informed and aligned with the hospital's goals and strategies.
Skills & Abilities
- Strong understanding of hospital operations, patient flow, and bed management.
- Skills in data analysis, systems thinking, and process improvement.
- Proficiency in using hospital information systems (HIS) and software, such as Meditech,

Qualifications
- Bachelor Degree in Health-related discipline
- Five (5) years in clinical experience in an acute care setting
- Three (3) years in leadership or management role
- Experience with Lean Six Sigma methodologies, project management, and data analysis in a healthcare setting preferred
