JobResponsibilities :
Toconsistently demonstrate in all relationships with familiescolleagues and partner professionals the Trust Values and the 6 Csof nursing of commitment competence courage communicationcompassion and care in the role of Health Visitor.
To develop aworkload profile identifying those groups within the populationwith specific health needs.
- To work closelywith the primary health care team participating in activities thataddress the health needs of the general practice case load familiesusing local childrens centers early education clusters and otherhealth partners.
- To formulate and agree withfamilys packages of care appropriate to need where necessaryopening active health and wellbeing files.
To monitor all openfiles in accordance with the records management policy closing whenpackage of care is complete.
- To carry outhealth promotion and disease prevention activities meeting andreporting on agreed key performance indicators whereappropriate.
- To give evidencebased advice onhealth issues to members of the local population in a way whichpromotes behavioral change and builds onstrengths.
- To work to current NICE guidelineson antenatal and postnatal mental health to identify support andreferral of women at risk of developing postnatal depression andthose who are mild to moderately depressed.
- Tomaintain quality standards of care using audit and other monitoringsystems to evaluate the service.
- To identifyvulnerable children in need of protection and refer to Social Careand Health following the Board policies in all cases of suspectedor actual emotional physical sexual abuse and neglect.
To follow updomestic violence notifications in line with Trustpolicy.
- To attend team around the familymeetings case conferences undertake and fulfil the responsibilitiesthat are appropriate to the role of the Health Visitor as agreed inthe multiagency child protection plan.
- Work inpartnership with families to plan and arrange appointments forclinics and home visits.
- To develop andmaintain effective working relationships with the families primaryhealthcare team and other professionals working in both thestatutory and voluntary organisations.
- Toensure all equipment used by professionals in clinic settings aremaintained and fit for purpose.
- To orderequipment for client use as identified following a healthcareassessment. To advise on safe fitting and appropriateuse.
- To promote and support immunisationprogrammes offered nationally to serviceusers.
- To ensure all written records are keptto the required NMC standard to be able to write letters ofreferral to other services and to produce complex reports for courthearings relating to child protection.
- To dealwith and support families through difficult family situations suchas illness domestic violence bereavement and childprotection.
- To have ongoing responsibility forthe management of a caseload including electronic recording datainputting and reports.
- To provide professionaladvice on issues relating to Health Visiting and promote anunderstanding of health needs among otheragencies.
- To access safeguarding caseload andclinical supervision in order to ensure a resilient workforce whichprioritises excellent safe compassionatecare.
- To participate in Nurse prescribingundertaking a full assessment of minor conditions appropriate tothe role of the Health Visitor and where indicated prescribe fromthe formulary appropriate to level of training and accesscontinuing professional development inprescribing.
- To convey complex information toother professionals and to clients and to work with otherprofessionals and clients in difficult circumstances such as childprotection terminal illness or bereavement.
