Position Description :
The Social Worker at the oversees and provides medically-related social services to ensure the mental and psycho-social well-being of patients in our skilled nursing facility.
This role includes managing a personal caseload, supervising a team of social workers, and ensuring compliance with federal and state regulations.
The Social Worker will implement Arizona Management System (AMS) methods for process improvement and maintain a focus on patient-centered care.
Key Responsibilities :
Plan, organize, and supervise the social services program, ensuring compliance with regulations and timely completion of assessments and documentation.
Supervise, train, and evaluate social services staff, handling staffing issues and performance reviews.
Oversee discharge planning
Manage grievances, ensure adherence to Patient’s Rights, and assist in budgeting and expenditure management.
Maintain a personal caseload, develop care plans, and participate in policy review and special projects.
Other duties as assigned.
Skills Required :
Knowledge of social services programs, regulatory standards, and principles of social work.
Experience with geriatric care, psychiatric issues, and community resources.
Strong supervision, leadership, and interpersonal skills.
Ability to manage caseloads, develop care plans, and work within budget constraints.
Experience Required :
At least one year of supervisory experience in a healthcare setting.
Experience in long-term care.
Ability to obtain and retain a valid AZ DPS Fingerprint Clearance Card.
Education Required :
Master’s degree in Social Work (MSW) and an active social work license issued by the Arizona Board of Behavioral Health Examiners.
