Sensity is seeking a Person Centred Planning and Navigation Specialist for an immediate opening for a full time role. Reporting to our Mentorship and Training Specialist, this role is an integral part of the teams surrounding the individuals receiving services from Sensity.
This position, based in Paris, ON, and will champion the person-centered approach to program delivery, the Person Centred Planning and Navigation Specialist will engage with people supported and / or their substitute decision makers, intervenors, and Intervenor Services Coordinators to facilitate the creation and implementation of goals and objectives.
Key Requirements :
- Facilitates the development and implementation of person-centered plans in collaboration with each person supported and / or their substitute decision makers, their Intervenor Service Coordinator, and their team of intervenors.
- Facilitates the development of each individual's It's My Life plan, including the creation of individualized goals and objectives.
- Works alongside the individual supported and their intervenors to create weekly plans that align with the individual's choices, values and goals, demonstrating the organizational value of "do with, not for".
- Participates in team meetings to discuss the progress of an individual's It's My Life plan and / or their program planning.
- Works with the Mentorship and Training Specialist to develop a training plan to foster the growth of the Intervenor Services team.
- Provides ongoing training to the Intervenor Services team related to the It's My Life program; calendar use; program planning best practices and other methods and techniques used in the provision of intervenor services.
- Delivers workshop-based sessions to educate intervenors services team and others regarding the use of specialized onsite resources such as Snoezelen and Music Rooms.
- Participates as part of a collaborative team in the development and delivery of activities available to service users.
- Meets regularly with ISC to discuss progress of program planning and goals.
- Informs the Mentorship and Training Specialists of emerging trends when identifying training needs within an intervenor services team.

Qualifications :
- College Diploma / Certificate in Human or Social Services, Intervenor or Sensory Loss program.
- Minimum of five (5) years experience in Deafblind and Intervenor Services.
- Experience in developing and evaluating person-centered plans.
- Good oral and written communication skills, with a strong client service orientation.
- Good presentation skills, with a focus on facilitating group meetings.
- Ability to build and strengthen relationships with internal stakeholders.
- Good time management and organizational skills.
- Adaptable and flexible with an ability to respond to changing work priorities.
- In-depth knowledge of MS Office 365 is preferred, including Word, Excel and Outlook and use of MS Teams for maximized collaboration of local and distance employees, clients, and programs.
- Demonstrates creativity and innovation as a visionary.
- French (written and verbal) an asset, but not required.
