We strive to provide quality wholistic health services by sharing and promoting traditional and western health practices to enable people to live in a more balanced state of well-being. The Centre provides services to Indigenous people including those who live on and off reserve status non-status Inuit and Metis within the Southwest Waterloo Wellington and Erie St Clair regions. SOAHAC has a mandate of ensuring that health services are accessible of high quality and are culturally appropriate. We are also mandated to build health care capacity within Indigenous communities. Currently we are seeking the services of a FASD Coordinator to join our interdisciplinary staff team to work out of one of our SOAHAC to the Integrated Care Manager in Owen Sound the FASD Coordinator is responsible for working with children and youth with FASD and/or suspected FASD and their families to develop strengths-based plans and support connections to appropriate diagnostic treatment and other supports services which meet the childs and familys needs. The individual will provide counselling and case management for Indigenous people who are living with mental health and/ or substance use issues. This role will take a lead in communications reporting requirements and ensuring decisions align with funding. The FASD Coordinator will be part of an interdisciplinary care team and will also be expected to work and partner with external service :Take the lead on creating guidelines for non-Indigenous client access in the Northern region to eliminate referrals with long wait times and travelReview the process used in Owen Sound and London for intake wait list management and follow up careManage all wait list and referrals made through both London and Owen Sound sites and act as the liaison between NavOn CSCN and SOAHACSatisfy reporting requirements to fundersUpdate roles and responsibilities of partners to ensure access for Indigenous clients is culturally appropriate and Indigenous ledWorking with children/youth with FASD or suspected FASD and their families to develop support plans based on individual strengths and needs and informed by the child/youth and familys vision goals and concernsSupporting information to families and connecting to supports and services on behalf of with families service providers and educators in supporting child/youth to build capacity and identify strategies to meet the needs of the child/youthMaintains relationships with all contracted service providers who provide fee for service assessments for the clinic and coordinates their contracts service meetings with families billing managementProviding information to parents caregivers and school personnel about behavioural symptoms associated with FASD and helpful approachesWorking with youth and their family and service providers across sectors to plan for transition to adult services if requiredFacilitate parent-to-parent connectionsParticipating in provincially-offered training to increase their own and their organizations capacity to support children and youth with FASD and their familiesRequirements:Diploma or Bachelor Degree in Social Work nursing special education or child and youth or social servicesMinimum 3 5 years of experience working with children youth and familiesExperience supporting FASD autism developmental disabilities mental health challenges and complex social needsDemonstrated knowledge of FASD and neurodevelopmental disabilitiesExperience in service coordination case management and system navigationExperience working with Indigenous communities using culturally safe trauma-informed Develops relationship with all Southwest Ontario Indigenous Communities Schools and Health Centres and acts as a single point of contact for FASD experience working with children/youth with FASD and their familiesUnderstanding of the services and supports available to children with FASD in Southwest OntarioDemonstrated ability to work independently and as a member of a cross-sector teamStrong interpersonal and communication skillsPeople leadership considered an assetTravel within Southwest Ontario as well as all SOAHAC sites as requiredWillingness to participate in Indigenous Cultural Safety Training teachings and ceremonies. Clean and current vulnerable sector police check as a condition of employmentValid government issued drivers license and access to a reliable vehicleUp to date immunization job may require additional responsibilities and duties as assigned by Southwest Ontario Aboriginal Health Access Centre.
