Recreation Programmer – In Home vacancy
Full-Time | 35 hours per week | Monday to Friday
Reporting to the Integrated Care Manager, Hospital Programs, the Recreation Programmer – In Home delivers individualized wellness and therapeutic recreation programs to frail and cognitively impaired older adults in their homes. This role takes a holistic, person-centered approach and is responsible for the design, development, implementation, and evaluation of individualized programs that support client well-being, independence, and quality of life.
Key Responsibilities
- Deliver one-on-one wellness activities in clients’ homes
- Support physical, cognitive, emotional, and social well-being through tailored programming
- Maintain accurate documentation and reports in electronic databases
- Collaborate with internal teams and community partners to achieve shared goals
- Manage time effectively to meet program and travel requirements
- Work independently while contributing to a multidisciplinary community care team
What You Bring to the Role
Background in Gerontology, Social Service Worker, Activation and / or Recreation , or an equivalent fieldMinimum two (2) years’ experience working directly with seniors ; one-on-one experience is an assetCertificate in Tiered Exercise, Functional Fitness, or other approved senior exercise training (or willingness to be trained)Recreation program leadership and coordination experience is an assetStrong organizational, time management, and planning skillsExcellent verbal and written communication, documentation, and customer service skillsAbility to actively participate as a team member and work collaboratively toward common goalsProficiency in MS Word, Excel, Outlook, PowerPoint , and database applications (e.g., GoldCare)Valid Ontario driver’s licence and access to a reliable vehicleCurrent First Aid and CPR certificationFluency in English and French and / or other languages is an assetWorking Conditions
Hybrid position with a base location at CHATS Head Office – 240 Edward Street, Unit 1, Aurora, ON, L4G 3S9Regular travel throughout York Region for in-home visits and program deliveryRequires flexibility, strong organization, and effective time managementAbility to work independently and as part of a larger community-based teamWhy Join Us?
