About this program

Founded in 2008, House Calls is a pioneering home-based primary care program committed to delivering exceptional healthcare services to frail, home-bound seniors in Toronto.

As an independent program, we operate within a collaborative framework alongside SPRINT Senior Care, who takes the administrative lead, and VHA Home HealthCare, who provides contracted Nurse Practitioner and allied health support.

Our goal is to support frail, homebound seniors living at home for as long as possible, providing comprehensive and dignified care that includes palliative services.

What will be your role?

We are seeking a dedicated and skilled Nurse Practitioner (NP) to join our dynamic interdisciplinary team of physicians, NPs, allied health professionals and team coordinators (House Calls Team).

The ideal candidate will bring experience in geriatric care, complemented by exceptional clinical and communication skills, enhancing our cohesive team dynamic.

Key Areas of Accountability :

  • Provide client and family-centered care independently and in collaboration with other team members to a small roster of homebound patients.
  • Provide urgent care and support to patients under the physicians’ caseloads.
  • Conduct comprehensive assessments, diagnose, treat, and formulate care plans for patients in a home-based setting.
  • Collaborate with internal and external community support services to provide integrated patient care.
  • Advocate for patients and their families, providing education and support for managing chronic conditions and improving overall health.
  • Make referrals and seek consultation for complex patient needs, while also providing mentorship, expertise, and consultation services to other members of the care circle.
  • Maintain accurate and timely documentation of patient care and progress.
  • Participate in regular team meetings to discuss patient care, program development, and continuous quality improvement.

Required Qualification :

  • Currently registered with the CNO as a Registered Nurse in the Extended Class.
  • Current RNAO or NPAO membership with LAP or equivalent.
  • Successfully completed a Primary Health Care Nurse Practitioner Program in Ontario or equivalent and hold a Master’s degree in nursing.
  • Strong clinical skills with the ability to work autonomously and within a team.
  • Recent experience in community nursing, geriatrics, and chronic disease management an asset.
  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.
  • Ability to manage time effectively and adapt to changing environments.
  • Proficient in electronic medical record (EMR) systems and technology used in home healthcare.
  • Commitment to continuing education and staying current with best practices in geriatric healthcare.
  • Valid driver’s license and use of a car on the job is required.
