What we are looking for :

The  Seniors Home Support (SHS) Program  was launched by the Eastern York Region North Durham (EYRND) Ontario Health Team to provide interdisciplinary primary care to homebound seniors who for medical, social or cognitive reasons cannot access office-based primary care services. As  Nurse Navigator , you will work closely with members of the team including family physicians, community paramedics, and allied health providers. The primary role of the Nurse Navigator is providing virtual telephone triage of health concerns for patients enrolled in the SHS program

The successful candidate will be available to work Monday- Friday, Days, 8 hr shifts, as per schedule.

Please note, schedule is subject to change based on operational needs.

Compensation :

Band J- $37.96 - $54.42 per hour

Location :   Markham and Stouffville, Ontario. This is not a remote job
