YOUR ROLEAT A GLANCE

As aPractice Specialist,youllplay a vital role in supporting children, youth, and families.Youllassess needs, create case plans, and coordinate family support services with a focus onpreservation and protection, helping children stay safely with their families whenever possible.

In this role,youllcollaborate with families, service teams, and community partners to design and implement plans that promotepermanency and positive outcomes.Youllalso provide expert consulting, training, and guidance on complex cases, and contribute to innovative solutions that meet the evolving needs of children, families, and communities.

HOWYOULLMAKE A DIFFERENCE

Assessment & Case Planning

Assess,evaluateand support familiess toidentifyrisks, needs, and strengths, then create case plans that promote stability and respect cultural values.

Provide crisis intervention, mediation, and conflict resolution, and coordinate case conferences and family meetings to achieve positive outcomes.

Connectfamilies to community resources, track progress, and adjust plans to ensure the well-being of children and families.

Case Management & Coordination

Manage an active caseload, work closely with families toimplementandmonitorcase plans, and advocate for children and families to ensure access to essential services and supports.

Collaborate with internal teams for guidance and transitions, track progress, adjust plans asneeded, and prepareaccuratereports and documentation.

Community Collaborationand Administration

Develop andmaintainstrong relationships with community partners and service providers to enhance support for families.

Support and leadinitiatives that prevent child intervention, strengthen family resilience, and ensure adherence to policies, cultural protocols, and legislation.

Provide consultation and training,participatein professional development, and contribute to program planning, evaluation, and service improvement.

Ensure accountabilitybymaintainingaccuratecase files and documentation, andassistwith CKAM Program reporting, data collection, and analysis.

WHAT YOU BRING TO THE ROLE

5 years or moreexperiencein Social Work

A Social Work diploma or degree

Knowledge of theMihtatakawSpiyAwasakWiyasiwwin Driftpile River Childrens Law

Valid Class 5 Drivers License and clean drivers abstract;

Police Information Check (Vulnerable Sector), Intervention Record Check, and Drivers Abstract will berequiredupon interview.
