POSITION SUMMARY :

  • To assist the Charge Nurse in delivery of care and supervision of residents requiring various assessments and treatments in the Home.
  • To monitor staff compliance with the Home’s policies and Procedures and applies appropriate counselling measures and / or reports to the Charge Nurse on duty, and / or the Resident Care Coordinator or designate so appropriate counselling / disciplinary measures can be applied when procedures are violated.
  • To function as a member of the Multidisciplinary health care team

DUTIES & RESPONSIBILITIES (not limited to) :

  • Observe and report clinical findings and changes in the resident’s condition to the Charge Nurse.
  • Administer personal hygiene, therapeutic measures and nursing care as directed, and according to policies and procedures.
  • Contribute to a suitable environment for all residents.
  • Assist in providing a therapeutic environment as identified in the Plan of Care for each resident.
  • Knowledge and skill to assist with diagnostic procedures.
  • Participate in the multidisciplinary team approach at Resident Care Conference and conducts the Resident Care Conference in the Charge Nurse’s absence.
  • Ensure the safety of nursing staff by example and supervise the use of appropriate equipment, techniques and procedures, as established by the Home.
  • Attendance at in / out of Home Programs, courses, workshops, in-services.
  • Assist with the admission of Residents, including the collection of data for the nursing history and assessment
  • Skin care for prevention of Pressure Ulcers and care of Pressure Ulcers under direction of the Charge Nurse.
  • Blood pressure, temperatures, pulse, respirations and Pulse Oximetry (oxygen saturation), weights and recordings.
  • Elimination : - Attending to resident’s toileting needs - Collection of specimens, urine, feces, sputum, etc. - Enema / suppository - Rectal tube for flatus
  • Transfer / discharge of resident, plus after care of bed, unit and Resident Record.
  • Hot and cold unsterile applications compresses, ice caps or collars, air ring.
  • Perineal and breast care.
  • Isolation technique.
  • Sterile and unsterile dressings (care of pressure ulcers), compresses, ostomies. Assist with active and passive exercises.
  • Document all procedures using the nursing process.

MINIMUM EDUCATION, EXPERIENCE & QUALIFICATIONS :

  • Must be registered and in good standing with the College of Nurses Ontario.
  • Must maintain BCLS certification.
  • Must be interested in professional self-development through in-services, continuing education and effective identification of learning needs and the use of resources.
  • Registered Practical Nurses when administering medications to Residents shall have completed the following :
  • A Practical Nurse program, which contains a Medication Administration course, as part of the core program. (Note : This is available in community colleges in Ontario);
  • OR - A Practical Nurse Medication Administration Certificate program provided in Ontario since 1985;

OR - A Medication Administration Certificate course provided out of province and which has been evaluated by the Director of the facility to determine if the content is equivalent to the course content offered in the Ontario Community College Practical Nurse Medication Administration Certificate Program.

  • Registered Practical Nurses who are responsible for giving Intramuscular Injections shall have completed the following : - A Practical Nurse Program which contains a Pharmacology Course that includes giving Intramuscular Injections as part of the core program (Note : This is available in community colleges in Ontario;
  • OR - A Practical Nurse Intramuscular Injection Course provided in Ontario;

OR - A Pharmacology Course with an Intramuscular Injection component or an Intramuscular Injection course provided out of province and which has been evaluated by the Director of the facility to determine if the content is equivalent to the course offered in the Ontario Community College Practical Nursing Program.

  • Registered Practical Nurses who are responsible for the completion of sterile dressings shall have completed the following : - A Practical Nurse Program which contains a Medical Asepsis Course as a part of the core program.
  • Note : This is available in the community colleges in Ontario);

OR - A Practical Nurse Medical Asepsis Certificate Course provided in Ontario; OR - A Medical Asepsis Certificate Course provide out of Ontario and which has been evaluated by the Director of the facility to determine if the content is equivalent to the course content offered in the Ontario Community College Practical Nursing Medical Asepsis Certificate program.

  • Registered Practical Nurses who are responsible for the completion of Urethral Catheterizations shall have completed the following : -A Practical Nurse program which contains a Urethral Catheterization Course as part of the core program (Note : This is available in community colleges in Ontario);
  • OR - A Practical Nurse Urethral Catheterization Course provided in Ontario;

OR - A Urethral Catheterization Course provide out of Ontario and which has been evaluated by the Director of the facility to determine if the content is equivalent to the course content offered in the Ontario Community College Practical Nursing Urethral Catheterization course.
