POSITION SUMMARY
The role of the Sherwood Park – Strathcona County PCN Family Practice Nurse is to provide comprehensive nursing services to the patient population of a family practice clinic within the Network. The nurse will work collaboratively with patients, family physicians and other team members to develop and implement care plans and provide follow-up with patients. The nurse will assess, manage and support patients to provide comprehensive care, education and guidance to patients with a variety of clinical conditions. Duties will vary depending on the clinic and patient population.
- 0.6 FTE - Permanent
- Mondays, Wednesdays and Fridays - 9 : 00 AM to 5 : 15 PM
- Clinics are located in the Griesbach area of Edmonton
KEY RESPONSIBILITIES
Include, but are not limited to the following :Work collaboratively within a family practice environment to enhance the delivery of primary care servicesNursing skills / tasks such as injections, dressing changes, suture removals, foot care, ear flushes, Pap tests, and chronic disease educationIdentify needs and set priorities with the patient, physician and other team membersAssist in developing and implementing patient self-management programsProvide relevant patient education to prevent and manage clinical conditionsCommunicate with patients to determine the next appropriate health services required, based on presenting concerns and test resultsProvide direction in accessing appropriate local and regional health programsImplement strategies for improving the quality and provision of care for the practice populationInitiate appropriate investigationsEnsure the maintenance of accurate records in line with practice policyOther duties as assigned to maintain seamless patient flow throughout office visitsProvide assistance to physicians as required and appropriate to the positionEDUCATION & EXPERIENCE
Licensed Practical Nurse diplomaFive years or more of related nursing experience, Primary Care experience a benefitCurrent CLPNA Registration, CPR, and BCLSExperience working in a team setting with other health professionalsSKILLS & ABILITIES
Expert knowledge of nursing processes and practices, including intervention strategies and therapiesStrong, effective written and oral communication skills, high level of perceptivenessAbility to prioritize, manage time effectively and be flexible in a very active work environmentRespect and understanding of confidentialityCustomer service orientationAbility to manage a complex changing environmentHigh levels of integrity and professional behaviorOTHER
Clinical experience in primary care nursing, community, gerontology and / or chronic disease managementAdvanced skills in health promotion, health screening, assessment and educationComprehensive knowledge of normal laboratory values and diagnostic imagingInitiative, accountability, sound decision-making and communication skillsDemonstrated ability and willingness to work in a dynamic environment that requires flexibility and balancing of competing prioritiesAdvanced skills in caring for patients with chronic, complex, and / or specialized needsBasic computer skills, ability to use Electronic Medical RecordsPERSONAL ATTRIBUTES AND COMPETENCIES
