What you do

Monitor, screen and interview clients to obtain information regarding their health condition and environment including safety screening, Opioid Use Disorder screening, safe supply screening, functional ability, living arrangement, and family support.

Report significant and unsafe behaviour, physical and cognitive changes, in accordance with standard procedures or specific directions, and reporting to Registered Nurse, Social Worker, or other members of the multidisciplinary team.

  • Arrange, plan, escort and accompany, as necessary through program clinical requirements, designated / marginalized individuals on medical appointments and errands, while providing emotional, moral support and assistance to further the enhancement or development of self-management skills.
  • Create linkages to other community resources and programs, contact relevant organizations / businesses / services, assist clients with processing forms.

Support and advocate for clients when identifying and accessing social services, such as income security, food, supplements, housing, social networks, while supporting the development of self-efficacy and self-care.

Act as a liaison between various agencies by methods such as updating designated team staff on services currently available and liaises with community agencies to provide information on services.

Provide Infectious Disease HIV or HEPC) information to community groups and health care providers regarding the sensitivity needed when working with vulnerable populations.

Support the provision of direct service components such as groups and education sessions with other program staff when necessary, including arranging meeting rooms and booking venues.

In conjunction with other members of the clinical team, attends, records, transcribes and distributes notes or minutes of meetings as necessary.

What you bring

Qualifications :

  • Graduation from a recognized Social Services Worker certificate program or an equivalent combination of education, training and experience.
  • Valid Class 5 vehicle license.
  • Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission’s 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
  • As a strong asset for consideration, we are looking for our successful candidate to have : Knowledge of social, economic, political and historical realities impacting indigenous communities and familiarity with Indigenous Cultural Safety and anti-racism and accompanying reports (BC DRIPA, TRC, etc.)
