Position Summary
Scope Navigator, Community Integration
The SCOPE Navigator leads the system navigation function of the SCOPE (Seamless Care Optimizing Patient Experience) program and collaborates with the members of the SCOPE interprofessional team and interdisciplinary supports in the North York community. This role provides leadership that helps planning, facilitate change, evaluate, and coordinate with all stakeholders. The Navigator serves as a single point of access for PCPs to access appropriate resources, and accomplishes this objective by triaging, assessing, planning, and communicating with the interdisciplinary team. Especially for complex patients who require care coordination across multiple sectors, the SCOPE Navigator will be the leader in developing individualized care plans with internal and external stakeholders to facilitate seamless care for PCPs and their patients.This position supports the hospital’s academic mandate to transform care through research and innovation, as SCOPE is an innovative program that acts as single point of access for PCPs to access a virtual interdisciplinary team. The program leverages innovation in virtual care and technology, aligning with the hospital’s mandate to transform care through innovation.

This is a Full Time Temporary Position of 18 months with 8 hour shifts, Days shifts in the Community Integration Department.
