POSITION SUMMARY :
PSW Integrated Comprehensive Care Program provides Hemodialysis Home Support services enhance the quality of life of SJHC clients by providing quality and compassionate care in to a select group of patients to receive Renal Care within their community such as a long-term care home or a client's home (home care).
The PSW-ICCP who provides Hemodialysis Home Support works cooperatively with the Manager of clinical programs, Program Assistants and other care givers in providing quality client-centered care, and in the effective execution of daily tasks.
PSW-ICCP adheres to daily care schedules and overall care plans.
These clients required more support in the daily management of their own care needs. The PSW will have enhanced learning and Skills Transfer opportunities in order to provide care to a certain client population.
CORE DUTIES AND RESPONSIBILITIES
- Attend initial patient home visit with Hospital nurse
- Attend Hospital orientation training is generally 6 weeks long with the hospital
- Verify the equipment of the machine before starting dialysis
- Follow care plan as per training
- Troubleshoot machine and report issues to supervising nurse
- Respond to emergencies and system alarms as per hospital procedures
- Provide care and monitor dialysis treatment as per the care plan
- Documentation of assessments, changes in patient conditions or treatment, critical incidents, and difficult events. Etc.
- Provide post-dialysis care as per the care plan
- Ensure the work area and equipment is clean and disinfected
- Following up and providing updates with the primary nurse 1-3 times per week
- Attend follow-up visits with patients in the home dialysis unit
- Administer medication at the discretion of the hospital
- Handle Bio hazard waste post-treatment wearing personal protective equipment (blood tubing, needles, and other access devices)
- Maintain treatment records
- Plan for emergencies regarding power outages, water issues, and emergency disconnection. Etc.

KEY EDUCAITON AND EXPERIENCE
- PSW Certificate from an accredited training institution, required
- 1-3 years PSW experience, preferred
- Current First Aid and / or CPR Certificate, preferred
- Training in gentle persuasive / non-violent crisis prevention, an asset
- Previous experience working in an Integrated Comprehensive Care environment, as asset
