Correctional Service of Canada’s (CSC) Pacific Regional Pharmacy is a federal government operated
hospital pharmacy providing pharmaceutical care to over 1600 patients at 9 institutions within British
Columbia. We are interested in employing a casual clinical pharmacist to join our team.
Days / Hours : Monday to Friday 07 : 45 hours to 15 : 45 hours
Location : Abbotsford, British Columbia
Salary : $65.00 / hour
Duties :
- Work within a multidisciplinary team and collaborate with other pharmacists, pharmacy assistants / technicians, physicians, nurses, specialists, and parole officers
- Provide in-person and virtual patient counseling and education
- Assess medication therapies to identify, mitigate, and document drug therapy problems
- Provide comprehensive oral / written recommendations to the healthcare team that are safe, effective, evidence-based, and patient-centered
- Use electronic medical records and provincial healthcare records to extend medication orders for intake into CSC as part of medication reconciliation
- Perform medication reviews to ensure continuity of drug care and Special Authority approval for discharge from CSC
- Participate in clinical pharmacists hospital in-patient bedside rounds
- Develop and deliver educational in-services and newsletters to the nursing team
- Attend meetings and serve as a member of the Regional Pharmacy and Therapeutics Committee
- Perform assessments and initiate medication orders for Minor Ailments Clinics
- Develop and lead medication management programs (ex. lab and therapeutic drug monitoring, deprescribing)
- Maintain patient confidentiality and comply with professionally accepted standards of practice
- Perform operational and dispensing duties if required
Qualifications :
- Final year towards graduation from an Accredited Pharmacist University in Canada
- Successful licensure with the College of Pharmacists of British Columbia by 2024
