Description

The Assistant Director of Care is responsible for planning, developing, implementing, evaluating, and carrying out nursing care services for Lawson Eventide Home.

KEY RESPONSIBILITIES :

Client Care Services

  • Coordinates and monitors the work activities and resources of assigned care services to achieve quality of client care.
  • Participates in the development of plans, objectives and strategies for care services which are aligned with overall organizational strategic directions.
  • Evaluates the performance of the care services through agreed upon objectives and measures.
  • Assesses, plans, implements, and evaluates client care collaboratively with the interdisciplinary team ensuring the delivery of client-centered care.
  • Works collaboratively with the DOC to decide on what admissions are accepted at Eventide Home.
  • Assist Director of Care in developing, recommending, and implementing operational policies.
  • Plan, develops, implements, and evaluates clinical care services in accordance with evidence-based practices.
  • Ensures consistency in programs and practices in client care services.
  • Ensures that the client care team complies with the organizational policies and procedures, continuing care health service standards, accommodation standards, relevant legislation regulations, accreditation standards and regional health authority requirements.
  • Responds, investigates and reviews concerns and complaints from clients, families, and staff.
  • Investigates and reviews all unusual incidents, critical incidents, and medication incidents.
  • Ensures that all intake / client agreements are met and provides direct support to staff as they work with clients who are not meeting the expectations of client agreements.
  • Ensures that client admissions, transfers and discharges are processed according to Policy and Procedures, and that the dignity of the client is maintained.
  • Ensures that frequent and ongoing opportunities for clients to engage in positive social and community activities are offered.

Team Management

  • Develops the nursing team through role modeling, ongoing coaching, mentoring and the provision of continuing education.
  • Assists as required, in regular performance appraisals and learning plans and addressing performance management in accordance with organizational policies.
  • Responsible for scheduling and appropriating staff hours within the care department and ensuring the hours are reported.
  • Maintains collective agreement compliance and effective ongoing working relationships with unions.
  • Liaises with RN Educator to ensure all staff has current education.
  • Ensures staffing complement maximizes resources to achieve appropriate client care.
  • Provides input into employee relations decisions which include hiring, terminations, and disciplinary decisions.
  • Ensure nursing staff completed the mandatory training; health care provider specific training e.g., Standard First Aid / CPR, infectious control, medication administration and e-learnings are completed within the set time frame and constantly review the skills and competencies of staff to adequately meet the requirement of the job.

Nursing Administration

  • Assists with doctors rounds when needed.
  • Ensures that the facility has established and implemented appropriate infection control procedures with ongoing monitoring.
  • Ensures that the master nursing rotations comply with union contract.
  • Ensures that safety and administration of medications is monitored, and follow-up is done on all medication errors to prevent recurrence.
  • Schedule and chair monthly nursing staff meetings and weekly case management meetings.
  • Ensure that Occurrence Reports are completed and processed as per Policy & Procedure.
  • Ensures that concerns with property and equipment are reported and that nursing station is organized, clean, and tidy.
  • Ensures the timely reporting of monthly / quarterly statistics.
  • Ensures that adequate program supplies are maintained.

External Interdisciplinary Liaison

  • Attends all meeting with interdisciplinary team.
  • Works collaboratively with case managers, physicians, and pharmacy.
  • Coordinates and leads family conferences and related meetings.

Perform other duties as required.

WORKING CONDITIONS :

  • This is a permanent full-time position based on 40 hours per week.
  • May be required to work various shifts including evenings, overnights, and weekends. Flexibility is required.
  • Work is mostly performed indoors.
  • May be exposed to medications, bodily fluids, waste, and other hazardous materials.
  • May be asked to carry a cell phone and may be expected to respond to calls as needed.
  • May be expected to travel between sites and group homes.
  • Programs support individuals that may exhibit behavioural issues.

QUALIFICATIONS AND EDUCATION REQUIREMENTS :

  • Completion of bachelor’s degree in nursing or equivalent.
  • Current registration as a Registered Nurse with the College of Nurses of Ontario and a member in good standing.
  • Current Standard First Aid and CPR level C or ability to obtain within the first 2 weeks of hire.
  • Will be required to be trained and certified in NVCI (non-violent crisis intervention and prevention) and First Aid.

EXPERIENCE AND KNOWLEDGE :

  • A minimum of 5 years of relevant experience including supervisory / team lead / management experience is required.
  • Previous similar experience in Long-Term Care with comprehensive knowledge of duties.
  • Knowledge in best practices of Infection Prevention and Control
  • Experience with coordinating committees for all required programs mandated by Ministry of Health and Long-Term Care
  • Experience creating and maintaining a pandemic / emergency plans.

SKILLS AND CAPABILITIES :

  • Demonstrate relevant understanding of, and compliance with, legislative knowledge of the Employment Standards Act, Human Rights Act, collective agreement and other applicable legislation as required.
  • Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures.
  • Strong written and verbal communication of information that enables effective engagement with the individuals we support, frontline staff, leadership, and external care partners.
  • Excellent communication skills with a diverse population whose communication may be limited, impaired, or atypical.
  • Excellent interpersonal communication skills and a collaborative team approach to create a working climate that is positive and mutually supportive.
  • Demonstrated commitment and ability to treat others with respect, dignity, and tact, as well as the ability to care for people with differing abilities.
  • Strong assessment skills.
  • Attentive to detail and values accurate information.
  • Ability to maintain the confidentiality of highly sensitive information is mandatory.
  • Demonstrated commitment to continuing professional development.
  • Demonstrated ability to be flexible and adapt to a fast-changing environment.
  • Valid Ontario Class G Driver’s License, personal vehicle required if there is no program vehicle (if applicable) and insurance, current copy of driver’s abstract that is satisfactory to The Salvation Army, in its sole discretion, may be required.
  • Attention to detail, problem solving and analytical skills.
  • Excellent communication (verbal and written) and comprehension skills.
  • Able to provide coaching and mentoring to staff.
  • Strong sense of integrity and confidentiality with professional ethics and a balanced sense of fairness and flexibility.
