We are recruiting for the following position : Sea Mar is a mandatory COVID-19 and flu vaccine organizationPosition SummaryWorks autonomously and collectively as an empowered staff member of Sea Mar Community Health Centers. Participates in the design, implementation, and ongoing delivery of services. This position primarily works with the PATH and Housing teams. This position is responsible for own caseload, providing the organization and community with consultations regarding the SOAR process, and for designing and implementing program standards essential to success of the CF PATH SOAR program.This position is also responsible for receiving referrals, screening for potential SOAR candidates, completing SOAR applications, and developing key contacts in the community to offer SOAR technical assistance.Duties and ResponsibilitiesAs a mission-driven organization, the core values of, and the services offered at Sea Mar are based on the belief that everyone deserves to be respectfully treated in a way that preserves dignity and enhances self-worth. - In consultation with the Manager of Housing Programs, organize, design and develop program structure, specific procedures and policies necessary to the operation and implementation of newly designed and / or modified programs, and operational and administrative forms necessary for the operation of the program, transmission of information, program satisfaction, and evaluation of the program effectiveness.
- Evaluate and monitor program operations in order to determine the soundness of the design and the effectiveness of the implemented procedures and policies.
- Develop new and / or review existing manuals pertaining to program policy, procedures, and operating instructions.
- Provide timely program reports to the Manager of Housing Programs and the Sea Mar management team on a regular basis.
- Provision of work-related services to help clients access the SOAR process.
- Assessment of the client's ability to obtain benefits, identifying specific problem behaviors and development of interventions to reduce or eliminate barriers to success.
- Development of an ongoing rehabilitation plan to help each client establish the skills necessary while participating in the SOAR process.
- Individual supportive case management to assist clients to identify and cope with the symptoms of mental illness that may interfere with their ability to obtain and maintain benefits.
- May provide clinical intervention to assist client in meeting treatment and level of care needs. Clinical interventions may include individual, family, and group modalities, and may include counseling, case management, medication management, vocational rehabilitation, system coordination, consumer advocacy, and crisis management.
- Ensure that treatment planning and interventions are client centered, culturally competent, and age appropriate. Consider all aspects of the client, their family, and their community's psychosocial, economic and cultural needs. Provide mental health care with respect for client confidentiality, safety and self-determination.
- Utilize community and natural supports to supplement services, with the intent to better integrate the client into their community, and to eventually graduate the client from services when possible and clinically indicated.
- Work collaboratively with co-workers and with personnel from other agencies with whom Sea Mar interacts.
- Complete all required documentation in an accurate, complete, and timely manner per organization policies and procedures. Takes the initiative to seek supervisory or collegial assistance with follow- through or methodology if problems with documentation quality or timeliness persist.
- Actively contribute to program development by participating in organization meetings, serving on working committees, and undertaking agreed upon independent projects for the improvement of the organization and enhancement of services.
- Maintain the safety, security, and confidentiality of clients and their families.
- Ensure own professional development. Maintain licensure and / or certification requirements. Contribute to the overall professional development of the organization. Contribute to the functioning of the organization as a training institution.Qualifications

- Ability to develop clear, concise and pertinent procedures, policies, instructions, and manuals necessary to the operation and implementation of the program.
- Demonstrated ability to effectively plan and organize work to satisfactory completion.
- Excellent written and oral communication skills.
- Computer proficiency (Word, Excel).Education, Certificates, Licenses, Registrations, and Medical Screening
- Requires Bachelor’s or Master’s degree in Counseling, Social Work, Psychology, or related field
- Possess or obtain an Agency Affiliated Credential
- Valid State Driver’s License and access to reliable car
- Auto Insurance
- Employee must authorize Sea Mar to obtain current driving record from the Department of Motor Vehicles (and annually thereafter) to determine employee’s driving eligibility. Driving record must meet Sea Mar’s vehicle insurance carrier’s minimum standards related to driving.ADDITIONAL REQUIREMENTS
