Position Summary

The Recovery Coach supports clients in developing the skills, routines, and plans required for sustainable recovery. Working alongside clinicians, addiction counsellors, and the broader care team, the RC focuses on behavioural change, recovery planning, and real-world application of treatment learning.

The role emphasizes doing, practicing, and integrating —not diagnosing or delivering psychotherapy.

Core Responsibilities

1. Recovery Planning & Transitions (Primary Ownership)

• Co-develop and maintain individualized Recovery Plans with clients
• Support goal-setting aligned with values, strengths, and readiness
• Lead transition-in and transition-out processes (Weeks 1 & 7 emphasis)
• Coordinate post-discharge supports (housing, employment, community services, outpatient care)

2. Skills Coaching & Behavioural Activation

3. Measurement-Based Care (Shared Responsibility)

4. Relational Support & Accountability

5. Team Collaboration & Documentation
