As a Care Coordinator , you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

Hours of Work
7.5‑hour shift (tour) between the hours of 8 : 00 and 20 : 00. Ontario Health atHome is accessible seven days per week, including weekends, and statutory holidays.
Availability Requirements
All new Part‑time B hires are required to be available for 6 weeks full‑time (37.5 hours per week) orientation (fully paid).
