How You Will Make an Impact

Reporting to the Program Manager of Community Support Services the successful candidate will be responsible for the provision of service to clients of the Second Stage and Transitional Housing programs. The candidate will assist households in the development of housing-based personalized care plans. As part of your role you will maintain a caseload and regularly engage with clients to monitor progress and provide support. Additional responsibilities include :

• Intake safety assessment and planning and assisting in the development of individualized care plans.
• Housing-based functions including housing searches viewings applications lease signings advocacy and referrals.
• Discharge planning
• Plan and participate in community events and public education opportunities
• Plan and facilitate groups / actives
• Maintain confidentiality with regards to clients / staff and Centres records
• Ensure adherence to occupational health and safety regulations corporate and program policy
