Licensed Practical Nurse - TPT

Triple Tee Medical Association is an inclusive and culturally responsive clinic, which provides meaningful and community-based healthcare.

Triple Tee Medical Clinic is seeking a Temporary Part-Time Licensed Practical Nurse.

Reporting to the Clinic Manager/Supervising Physician, the LPN will be responsible for providing safe, compassionate and client-centred nursing care to clients in accordance with their individual needs, established care plans, organizational policies and procedures, and the professional standards and requirements applicable to LPN practice in Saskatchewan. The LPN will work collaboratively with clients, families, caregivers, physicians, registered nurses and other healthcare professionals to provide quality nursing care, monitor client health status, support treatment plans, administer medications and treatments within the LPN's scope of practice, and promote client health, independence and well-being.

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support, that will deliver better access to primary care in the province of Saskatchewan.

Responsibilities:

  • Complete nursing assessments within the LPN scope of practice.
  • Monitor vital signs, symptoms, and changes in health condition.
  • Recognize changes or deterioration in a client's condition and report/escalate appropriately.
  • Participate in developing and evaluating individualized care plans.
  • Provide safe, competent, compassionate, and client-centered nursing care.
  • Promote client dignity, privacy, independence, and safety.
  • Perform nursing procedures within your LPN scope of practice and competence.
  • Administer medications safely and accurately according to policies and scope of practice.
  • Monitor medications for effectiveness, adverse reactions, and complications.
  • Educate clients and caregivers about medications, treatments, and health conditions.
  • Perform clinical procedures, such as wound care, dressing changes, blood glucose monitoring, injections, catheter care, and specimen collection, when authorized.
  • Monitor and manage clients with chronic health conditions as appropriate to the role.
  • Provide health education and promote healthy lifestyle choices and self-management.
  • Communicate changes in client condition promptly and accurately to the appropriate healthcare professional.
  • Collaborate with RNs, physicians, and other healthcare professionals to provide coordinated care.
  • Maintain accurate, complete, and timely nursing documentation, including assessments, treatments, interventions, and client responses.
  • Maintain confidentiality and protect client health information.
  • Follow infection prevention, medication safety, and workplace safety procedures.
  • Report incidents, medication errors, adverse events, abuse, neglect, and safety concerns appropriately.
  • Advocate for clients' rights, dignity, preferences, privacy, and informed decision-making.

Qualifications:

  • Completion of an approved Practical Nursing Diploma program
  • Current registration and good standing with the College of Licensed Practical Nurses of Saskatchewan (CLPNS) with the ability to practice within the LPNs legislated and professional scope of practice.
  • Experience in community health, case management, or a Primary Health Care setting is considered an asset.
  • Demonstrated knowledge of primary care and chronic disease management as per CLPNS guidelines and employer-approved protocols.
  • Proficient in EMR systems including documentation, lab review, and medication reconciliation.
  • Strong clinical observation and assessment skills.
  • Strong interpersonal communication skills with the ability to work collaboratively in an interdisciplinary team.
  • Proven ability to prioritize, manage caseloads, and maintain confidentiality and professionalism.
  • Commitment to ongoing professional development and adherence to evidence-based practices.
  • Participate in continuing education and professional development.
  • Follow applicable professional standards, legislation and clinic policies.
  • Demonstrate accountability for professional decisions and nursing practices.
  • Commitment to client-centered care.
  • Ability to respond appropriately to changing client needs.
  • Maintain availability to work scheduled shifts and provide availability to work additional shifts when mutually agreed upon according to operational and clinic requirements.
  • Participate in required staff meetings, training, and education sessions.
  • Adhere to clinic policies and procedures.
  • Maintain reliable attendance and punctuality.
