Key Responsibilities
- Provide professional social work and crisis intervention services to seriously mentally ill clients.
- Develop and implement creative treatment and recovery plans, discharge planning, support services and programs responsive to clients’ needs.
- Engage in client-centered care.
- Collaborate with the inter-professional team.
- Maintain appropriate documentation and data collection, in accordance with hospital, college and legislative standards.
- Participate in the development and delivery of educational and information sessions for clients, families, service providers, and the community.
- Participate in Social Work meetings, peer consultation and committees.
- Keep up to date regarding professional standards by reviewing literature, reading articles, attending educational events and networking with colleagues
- To work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the ROHCG.
- To ensure a work environment that is conducive to the ROHCG Harassment Free policy.
Qualifications
- Bachelor of Social Work from an accredited university.
- Three years’ experience in a mental health setting.
- Member in good standing with the Ontario College of Social Workers and Social Service Workers.
- Knowledge, skills and training in concurrent disorders.
- High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
- Knowledge of community resources, corrections, and applicable legislated acts (MHA, PHIPA, The Health Care Consent Act, .
- Understanding of recovery-based treatment and programs of clients with severe and persistent mental illness.
- Demonstrate ability to work both independently and in a professional inter-disciplinary team environment.
- Strong advocacy & conflict resolutions skills.
- Strong organizational skills and work ethic, with excellent oral and written communication skills.
Language Requirements
English level A- is mandatory in oral expression, oral comprehension, reading comprehension and written expression.
Salary Range: 43.31 to 48.72 per hour
