House Calls delivers comprehensive in-home healthcare to vulnerable seniors who experience barriers accessing traditional office or clinic-based care.A partnership between primary care, SPRINT Senior Care and VHA Home HealthCare, our integrated team includes Physicians, Nurse Practitioners, Social Workers, Occupational Therapists, and Physiotherapists, supported by in-office Coordinators.The House Calls team strives to improve the health and quality of life for all our patients. Our care plans include comprehensive and ongoing primary care, chronic and acute disease management, medication administration, in-home interdisciplinary assessments, ongoing case management, and assistance with navigating the healthcare system. The House Calls team also facilitates safe discharges and transitions from the emergency department and hospital inpatient beds to enable safe recovery and care in the home setting.Find more about this program on: www.seniorshousecalls.caVHA's Physiotherapist is responsible for providing physiotherapy consultation, assessment and planned interventions for diverse client groups living in Toronto. The physiotherapist will work within a continuous quality improvement framework that reflects VHA’s vision and mission of services.
