Position Overview Reporting to the Director Health Services (DHS), Neonatal Pediatric Critical Care (NPCC), and supported by the Operations Director of Indigenous Health and DHS, Child Health Program. The Indigenous Nurse Navigator is a registered nurse that will focus on culturally safe care, coordination, advocacy, family support, and navigation through a complex multidisciplinary care system for First Nations, Métis and Inuit Peoples that move through NPCC and Child Health. The Indigenous Nurse Navigator has knowledge of Indigenous context, culture, the impacts of colonization and racism, and gaps and barriers to accessing care in the NPCC and Child Health Programs. This position will provide family-integrated guidance and support and will empower Indigenous patients and families throughout their journey through NPCC and Child Health as collaborative decision makers with all members of the health care team. Through case management and comprehensive assessment, the Indigenous Nurse Navigator can assess, recognize and address a wide range of physical, psychosocial, emotional, and practical needs. The incumbent will work utilizing standard operating procedures, directives and guidelines, along with their collaboration with the multidisciplinary teams. The incumbent will support patients navigating throughout their journey along the healthcare system by creating plans that address their specific needs and the needs of those included in their circle of care. Approaches will include facilitating access and connections to supportive services as well as participating in discharge planning to ensure a successful transition from hospital to home.

Experience

Lived experience and understanding of Indigenous healthcare priorities, services, and challenges alongside Indigenous culture, protocols, traditions, and values, both within communities and organizations in Manitoba.

Experience in or knowledge of Indigenous Services Canada (ISC) and the relationship between ISC and the Manitoba health care system.

Professional experience caring for neonatal and/or pediatric patients and families in tertiary care setting.

Community/Advance practice nursing in Indigenous Community.

Experience with community and/or public health healthcare delivery and/or case management, an asset.

Experience in working with equity‑deserving communities.

Must have satisfactory employment record.

Education (Degree/Diploma/Certificate)

Bachelor’s degree in Nursing from an accredited educational institution required.

Master’s degree preferred.

An acceptable equivalent combination of education and experience may be considered.

Certification/Licensure/Registration

Must be registered or eligible for registration with the College of Registered Nurses of Manitoba (CRNM).

Qualifications And Skills

Knowledge and appreciation of the unique history, cultures, and rights of Indigenous Peoples in Canada is required.

Knowledge or familiarity with Indigenous health issues, population health, and health status of citizens in Manitoba.

Knowledge of the Manitoba health care system, federal health care system, provincial and federal governance, and leadership models.

Knowledge or experience with Child and Family Services system.

Ability to cultivate and develop inclusive and equitable working relationships.

Strong communication skills, within a diverse and multidisciplinary team.

Microsoft programs/EPR/EDIS.

Physical Requirements

The incumbent must be available to work evenings and weekends as required.

Up to date immunization record.

Must be legally able to work in Canada.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
