The

BSS-MSTs

improve the health care services of older adults, their families and caregivers. The responsive program supports those who live and cope with behaviours/personal expressions associated with dementia, mental illness, addictions and other neurological conditions, when they require it and wherever they live, at home or elsewhere. Behaviours/personal expressions are a response, often intentional, to something important to our clients. Persons may use words, gestures, or actions to express something important about their personal, social, or physical environment. Behaviours/personal expressions such as wandering, physical resistance and agitation can be common for individuals living with dementia, mental illness, addiction or other neurological disorders. They can be very distressing to the older adults themselves and to their families, caregivers and other support providers. Job Summary

As a

Case Manager, Behavioural Support

you will be part of a Behaviour Support Team that provides direct care, education and support to seniors who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions and who are ALC or at risk of becoming ALC in the acute care settings of the Ontario Health Central or Toronto Central area. You will assist in the creation and coordination of a behaviour support network, you will provide direct service to support the person served, family members and/or care‑givers to mitigate escalating responsive behaviours. A key role of this position is to coordinate and support the transition of person served as they moved to their next experience. What You Will Do

Develop and implement individualized Behavioural Support Plans aimed at improving quality of life and reducing responsive behaviours. Conduct behavioural assessments and maintain accurate, timely documentation and reports. Act as a liaison and advocate, collaborating with clients, families, caregivers, and multidisciplinary teams. Provide training, consultation, and ongoing support to staff, families, and caregivers. Offer supportive counselling and connect clients to appropriate health care services and community resources. Deliver crisis intervention and assist in creating effective crisis management plans. Build respectful, trusting relationships using recovery-oriented and psychosocial rehabilitation principles. Participate in team meetings, care conferences, and interdisciplinary collaboration. Maintain strict confidentiality and adhere to organizational policies and procedures. Work primarily within hospital settings, ensuring compliance with health and safety protocols. Engage in required training and contribute to the Behaviour Support Resource Team. Perform additional duties as assigned by leadership. What You Bring

Completion of a diploma/degree in Social Services/Human Services or equivalent; Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions; An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with persons served on goals/priorities; Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills; Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive training; Ability to collaborate effectively with other members of inter-professional care teams, persons served, family members and community team members; Good oral and written communication skills and computer literacy, including MS Word and Excel; Counselling and responding skills; Creativity and flexibility to approaches; Knowledge of community resources and supports; Ability to work effectively as part of a team and interact with workers from a variety of agencies; Strong assessment skills; Understanding of cultural competency and framework Valid drivers’ license and access to own vehicle is required; Work Environment

Staff will be required to work in clients’ rooms and may therefore be exposed to secondhand smoke. May also be required to be on-call as emergency contact for after hours. Frequent travel within the Central LHIN will also be required. Occasional evening and weekend hours may be required What We Offer
