The Post Majority Care Case Worker provides case management, service coordination, and transition planning for First Nations youth and young adults up to age 26 who are aging out of, or have aged out of, care. Working collaboratively with youth, families, and community partners, the Case Worker delivers culturally safe, trauma-informed, and strengths-based supports that promote independence, well-being, and connection to culture and community. The incumbent works in accordance with applicable legislation, funding requirements, and Batchewana First Nation policies and procedures.

Duties And Responsibilities

Provide case management, facilitate programs and transition planning for BFN youth and young adults up to age 26 who are aging out of, or have aged out of, care.

Complete assessments and develop individualized support plans that promote independence, well-being, and successful transitions to adulthood.

Coordinate services and connect youth with appropriate supports, including housing, education, employment, life skills, financial assistance, health, mental health, and cultural programming.

Advocate on behalf of youth and assist them in accessing community, provincial, federal, and Indigenous programs, services, and resources.

Support youth in strengthening connections to their families, culture, language, community, and traditions, where appropriate.

Monitor progress, review support plans, and adjust services as needed to meet changing needs.

Build and maintain collaborative relationships with internal departments, community partners, service providers, and other agencies to support coordinated service delivery.

Deliver services using a culturally safe, trauma-informed, strengths-based, and youth-centred approach that reflects the values of Batchewana First Nation and the Seven Grandfather Teachings.

Participate in cultural activities, ceremonies, training, and community events to strengthen cultural knowledge and community connections.

Maintain accurate, timely, and confidential client files, case notes, reports, and other required documentation.

Ensure work is completed in accordance with applicable legislation, funding requirements, and BFN policies and procedures.

Participate in team meetings, professional development, and mandatory training while contributing to a positive, respectful, and collaborative work environment.

Required Skills Minimum Education

Bachelor's Degree in Social Work or relevant Human Services Degree preferred

Community College Diploma in Social Services may be considered in combination with experience.

Minimum Experience

Minimum of two (2) years of experience coordinating and managing social programs and services

One (1) year of direct service experience with youth and young adults up to the age of 26 who are aging out of, or have aged out of, care.

Experience working with Indigenous people, organizations, and communities

Equivalent combination of education and experience may be considered
