Job description
Your Opportunity: You’re an experienced leader with a clinical background who’s about thinking bigger – solving problems and making things better on a systems level. Assisted Living Alberta (ALA) is seeking a person like this who combines clinical or social work experience with leadership ability for the role of Program Director for the Alternate Level of Care Centre of Expertise (ALC/COE). The Program Director, ALC/COE is a management role responsible for implementing best practices for transition services, addressing systemic patient transition issues, and resolving complex transition cases. In this role, you will deliver advanced practice support and contribute ALC and transition expertise that informs and operationalizes provincial quality standards that strengthen clinical decision making and improve patient flow, transition experience, and outcomes. Assisted Living Alberta (ALA) provides Albertans with the support they need through a comprehensive assisted living system that offers a full range of services, including medical and non-medical supports, continuing care homes, home care, community care, and social services. ALA is focused on delivering care more effectively and consistently across the province by enhancing home and community services, expediting access to supportive living environments and continuing care homes, and ensuring Albertans and their caregivers are well-informed.

Description: The Program Director, ALC Centre of Expertise (ALC/COE), is a management role accountable for implementing best practices for transition services, convening ALA and system stakeholders to address systemic transition issues, and resolving complex transition cases. Reporting to the Executive Lead, Client Navigation and Integration, this role delivers advanced practice support and contributes Alternate Levels of Care (ALC) and transition expertise to inform and operationalize the provincial standards set by the Planning, Performance and Quality (PPQ) leader, fostering a culture of continuous learning across the portfolio. As Program Director ALC/COE, you will provide clinical leadership for ALC and care transitions across Alberta. Leading a team of over 10, including subject matter experts, clinical leads, and educators, you will implement and maintain standardized protocols, tools, and best practices for ALC transitions and broader discharge planning, in collaboration with key stakeholders. As Program Director, you will ensure prudent use of fiscal, financial, and human resources. In this role, you will apply standardized, evidence-informed practices across the portfolio, overseeing advanced training to embed best practices into frontline delivery using actionable clinical tools, pathways, and protocols that strengthen decision-making and improve patient flow and transition outcomes.

Required Qualifications The successful candidate for this role will possess a master's degree in health administration, social work, public health, or a related field. You have at least three (3) years managerial/leadership experience in healthcare service delivery and minimum of five (5) years experience in ALC management, transition services, or discharge planning. A combination of education and relevant experience may be considered.

Additional Required Qualifications The successful candidate has experience leading subject-matter experts and drive continuous improvement in care transitions with demonstrated expertise in developing training programs and best practice frameworks. Experience in knowledge management, standards development, and evidence-based practice is required.

Preferred Qualifications Experience in Alberta's ALC strategy and continuing care transition landscape is preferred, with familiarity with social work practice in healthcare settings a strong addition. Certification in quality improvement, adult education, and change management is an asset.
