Program Overview – First Steps to Home, Supportive Housing – Community Care Unit

This position is responsible to the Manager Supportive Housing Program and/or Supervisor, Supportive Housing Programs for support services for individual clients and their communities.

This position offers services to tenants of Supportive Housing Programs, living in their own bachelor-style unit. All participants have prolonged mental health and/or addictions; have experienced homelessness and who are working to maintain affordable and sustainable housing. This position is expected to carry out the services in accordance to WoodGreen’s policies and procedures, procedures, Ministry of Health, and health-related authorities, and all relevant legislation.

What You Will Do

The Food Access Coordinator, reporting to the management team, will play a crucial role in ensuring residents have access to and knowledge about healthy food choices. Working collaboratively with a multidisciplinary team; they will coordinate services with the kitchen team, PSWs, case managers, and client support workers.

The primary responsibilities of the Food Access Coordinator include coordinating the promotion and implementation of participant involvement in various food programs. These programs may include a food bank, cooking workshops, gardening initiatives, and similar activities aimed at enhancing food access. Additionally, the coordinator will be responsible for upholding health and safety standards and fostering a welcoming environment.

Specific Accountabilities:

Conduct assessments to determine individual nutritional needs and develop an individualized plan. Implement food services program within budgetary constraints. Coordinate, monitor, and ensure the proper ordering, receiving handling, and storage of both purchased and donated food items. Maintaining and cultivating strong relationships with food banks and other supportive agencies. This includes networking and conducting outreach to a diverse range of food providers. Coordinate and operate food banks and similar programs to ensure access to nutritious food for seniors. Develop and maintain gardening and similar programs. Support front-line staff in the supervision of volunteers and placement students. Provide direction and coordination of team development needs and opportunities/initiatives. Maintain a safe and welcoming environment through excellent customer service and conflict management. Provide one-on-one coaching to tenants on nutrition, meal planning, and grocery shopping. Develop, enhance, and maintain systems including all records and schedules as required. Tracks daily meals served and other relevant statistics for funders and program development with the assistance of Food Access Assistant and Drop-In staff. In conjunction with the Director and other Drop-in Coordinators, monitor and ensure the continued development of program policy and procedures. Develops and maintains strong working relationships with other internal and external services and programs. Ensure the purchase and maintenance of program equipment and supplies. Be prepared to work in environments where they might be pest control and hoarding issues Be prepared to work in environments where there may be death and dying Be prepared to work in environments where there may be drug use And other duties as directed by management team

What You Bring to the Team

Bachelor’s degree in Nutrition, Public Health, a related field or work 2+ years’ experience working with individuals’ mental health, addiction, street-involvement, and community based nutrition programs, preferably seniors
