Dietician

The Registered Dietitian provides dietetic and nutritional care to clients and community members, using a holistic approach to health promotion, illness prevention and chronic disease prevention and management. The Registered Dietitian also participates in the development, implementation, monitoring and evaluation of nutrition and food security programs and services for individuals, couples, groups, families and the community. The Registered Dietitian is part of the Interprofessional Primary Care Team. Responsibilities

1. To accurately assess and assist in arranging client-centered nutritional care, counselling, and education for clients. 2. Be responsible for using advanced clinical reasoning skills to ensure a high standard of assessment, goal setting, treatment and evaluation of patients based on the principles of client centered care. 3. Plan, implement, and evaluate community-based nutrition programs, workshops, and cooking demonstrations. 4. Provide individual and group counseling for healthy eating, disease prevention, and management (e.g., diabetes, cardiovascular health). 5. Review existing resources and develop appropriate resources which meet the diverse needs of the community. 6. Research, reviews, interprets and disseminates current nutrition information and best practices, through workshops, media and community calendar(s). 7. Collaborate with local schools, food banks, health centers, and community groups to improve access to healthy food. 8. Develop and deliver evidence-based educational materials and resources on nutrition. 9. Assist in the development of balanced, culturally sensitive meal plans for the Home and Community Care program, tailored to clients’ nutritional needs and dietary restrictions. 10. Provides direct patient care and education to clients to facilitate their self-management of prevention of chronic health conditions (e.g. diabetes, cardiovascular health) and complications. 11. Identify nutrition problems, assess the nutritional status of patients, develop nutrition care plans, and evaluate/monitor effectiveness of nutrition interventions in relation to the management of diabetes. 12. Work collaboratively with the Diabetes education team to facilitate a team-based approach to care. 13. Develop, deliver, and evaluate initiatives appropriate to the needs of the community and or group (e.g. grocery store tours, cooking sessions). 14. Conduct nutritional assessments for individuals and groups and monitor the outcomes of community programs. 15. Collaborate with other healthcare professionals to develop, implement, and evaluate healthy nutrition policies. 16. Work with community agencies and groups to promote access to sufficient, safe, nutritious, and personally acceptable food for people of all ages. 17. Maintain accurate, timely, and confidential client records 18. Prepare and submit statistical reports. 19. Remain abreast of new research by attending dietary workshops, seminars, conferences, and classes. Qualifications

Bachelor and/or Master’s degree in Nutrition, Dietetics or related field. Member in good standing of College of Dietitians of Ontario (CDO) Current Certified Diabetes Educator certification preferred Minimum of two years’ recent work experience as a Dietitian is preferred Advanced knowledge of principles, processes, and practices in Dietetics and/or nutrition. Ability to understand and work harmoniously with non-professionals and professionals but also be self-directed and able to work independently. Excellent ability to manage high-stress or highly emotional situations in pursuance of positive outcomes. Proficient with PC operation, including word processing, spreadsheets, and proprietary healthcare software. Excellent oral communication skills and written communication skills. Knowledge or training in adult education techniques Sound judgment and able to execute decisions efficiently. Submission of CPIC & Vulnerable Screening Utilize discretion, tact, diplomacy, and respect when dealing with others. Ability to respect and adapt to diverse social and cultural backgrounds.
