Yes No LAMP Community Health Centre is a multi-service, community based charitable organization that provides a wide range of programs and services to improve a person's health. Under the direct supervision of the Manager, Primary Care Services and as a member of an interdisciplinary team, the physiotherapist provides service through individual treatment, groups, or consultation to families & caregivers at the centre or in the community. The Physiotherapist will ensure that all organizational policies are followed; ensure the provision of high-quality services to clients in keeping with LAMP values, Hub principles, and the health priorities of BAC communities; and reflect the high-quality work and integrity of LAMP and the Hub collaborative at all times Primary Responsibilities The Physiotherapist will assess, identify and analyze strengths and impairments related to physical activities. The therapist will then develop and implement a therapeutic program involving appropriate, goal-oriented interventions, working toward functional goals agreed upon with the patient. The Physiotherapist is an on-site resource for the team for issues related musculoskeletal pain and injury. Client Management: includes assessment, treatment, and discharge, determining short- and long-term goals with clients, implementing treatment plans. Communication: recording relevant assessment and treatment data in electronic health record, attending case conferences as appropriate, consulting with other members of clients’ healthcare team. Working effectively with patients to achieve common objectives. Acting to create and sustain highly motivated patients to achieve common objectives. Administration: managing time, prioritizing and maintaining schedules and appointment dates, assisting with planning and evaluating programs, participating in chart audits and quality improvement initiatives, maintaining current professional membership and licensure, promote evidence-based practice. Adapts to and works effectively with various individuals in a variety of situations or adapt one’s approach to accommodate changes in the situation. Understanding and appreciating different and opposing perspectives on an issue. Ensure clinical supplies are available when required. Supervision: supervising and overseeing activities of any Physiotherapy Assistants or students that may join the team; legally responsible for their actions Other relevant duties as assigned by the Manager. Current registration, in good standing, with the College of Physiotherapists of Ontario. Well-developed professional skills which include: the ability to work within an interdisciplinary team setting, the ability to collaborate and consult with clients and community caregivers, excellent assessment, treatment and consultative skills, the ability to communicate clearly and effectively both orally and in writing and the ability to work in a variety of settings, At least three years’ experience, preferably in community-based care. Demonstrated ability to create and implement clinical initiatives. Strong clinical reasoning and caseload management skills. Demonstrated ability and willingness to maintain professional standards of practice and clinical competencies according to College guidelines and participate in continuing education in order to upgrade skills and keep abreast of new developments in adult and geriatric rehabilitation. Demonstrated ability to work in a changing environment which demands high productivity, organizational skills and flexibility. Demonstrated ability to adapt to changing referral patterns based on needs of diverse clients, community and centre priorities. High level of creative problem solving, decision making and interpretive skills. Versatility and adaptability to fulfill the multi-dimensional role of clinician, educator and administrator. Ability to manage own time, resources, flow of work to benefit self, staff and clients Knowledge of LAMP Community Health Centre, and priority populations we serve across Etobicoke and the Peel Region.
