Nature & Scope

YMCA BC is the lead agency for Foundry Prince George, working with partners to deliver responsible and meaningful services for young people 12-24 and their families. Foundry is a provincial network of integrated and innovative wellness centres working to transform access to care. Foundry’s mission is to support young people to lead good lives. To this end, Foundry centres offer mental health and substance use services, primary care, social services, and youth and family peer support.

The Peer Support 3 (PS3) title replaces the former peer support life skills worker title. This full-time role is a significant step beyond the PS2 role and has greater responsibilities and expectations of staff.

Reporting to the Foundry Peer Support Coordinator, the PS3 utilizes their own professional and lived experience with mental health navigation or problematic substance use management, to improve social determinants of health for young people requiring assertive support. The PS3 collaborates with the youth and Foundry PG’s multidisciplinary team to determine appropriate care to meet individuals “where they are at” and remove barriers. The PS3 provides youth with advocacy, education, goal setting, and assertive support from a lived experience perspective to assist youth in effectively obtaining service internally at Foundry and externally in community. The PS3 assists youth living with problematic substance use with psychosocial support, health care navigation, goal setting, information, transportation, mentorship, resources, and demonstrations.

Working closely with Foundry’s Concurrent Disorder Clinicians, the PS3 contributes to case planning and execution, follow up, and risk assessment, with strong contribution to documentation, data collection and program development. Services are provided on site, and through outreach to the streets, clinics, shelters, and personal housing. As a main point of contact for many young people and their families accessing Foundry, you will provide non-judgmental and compassionate, trauma-informed service. Within the context of a client and family-centred model of care and the mission and values of Foundry BC and the YMCA, you will promote cultural safety, harm reduction, hope inducing strategies, and respect among young people, families, visitors, and team members.

Major Responsibilities- in this position, you will:

• Function as a direct resource support to youth 12-24 working closely with Foundry concurrent disorders clinicians or Foundry primary care providers
• Strong initiative taking as a key member of Foundry’s integrated and multi-disciplinary team
• Build rapport and trust with youth impacted by trauma, loss, bigotry, or social exclusion
• Consistently demonstrate strong personal and professional boundaries and offer non-judgmental support
• Maintain personal well-being and stability to be consistently ready to support others while on shift
• Complete and maintain related records and documentation in electronic medical records and data systems including service information, progress reports and notes, and care plans
• Use Toolbox and MOIS proficiently and consistently to contribute to Foundry data collection, ensuring encounter information is timely, accurate, complete, and sufficiently detailed to communicate relevant information to other team members
• Independently and consistently check email, complete tasks for ADP, and complete assigned learning centre tasks for YMCA BC and Foundry
• Assist youth with the activities of daily living including time management, organization and interpersonal communication to help improve their social determinants of health
• Assist and mentor youth to improve their daily living situation including support in the areas of housing security, social connection, education, employment, financial security, community engagement, personal skills development, social skills development, and holistic wellness
• Accompany youth to appointments in the community or onsite at Foundry PG, including transporting youth in personal vehicle
• Represent Foundry and YMCA in the community and at meetings
• Actively participate in YMCA and/ or Foundry team meetings, huddles, communities of practice, and training opportunities
• Communicate relevant information about youth and their care in a timely way to other members of the Foundry team through appropriate channels
• Develop, lead, or co-facilitate group services to promote skill development, mental health and substance use recovery, and relapse prevention
• Provide support to aid with reducing youth’s distress, improve or maintain functioning and enable independence and active participation
• Work collaboratively with team members to meet pre-established goals and objectives, facilitates recovery-based peer support meetings and activities
• Participate in team meetings, case conferences and organizational initiatives
• Work as a liaison between Foundry PG, youth accessing the centre, and other YMCA and Foundry services or supports
• Mentor PS1s, PS2s, and volunteers; model positivity and professionalism
• When directed, assist PS1s and PS2s in areas like Toolbox and MOIS, completing forms, and understanding resources
• Ensure safety issues, near misses, or potentially unsafe situations are reported to leadership promptly to mitigate risks
• Work with leadership to manage challenging situations (i.e.: develop plan for supporting youth you know personally from the community)
  
  

Qualification-you bring:

• Lived experience of critical issues relevant to young people accessing Foundry, including mental health, substance use, trauma, growing up in government care, newcomer experience, etc.
• One of the following:Diploma in community social services or related field, orTwo years of recent work experience with an emphasis on mental health, addictions, homelessness, and/ or social services, including one year of service planning experience, orEquivalent combination of education, training, volunteering and lived experience and eligibility for completion of a recognized youth peer support training program and willing to self-identify and share experience of recovery
• Completion of peer support training through Foundry Central or other provider
• Additional education, training, or experience a distinct asset
• Personal understanding of and alignment with the mission, vision, strategic outcomes, and values of the YMCA and Foundry
• Current First Aid and CPR certification, opioid poisoning/ overdose naloxone response a distinct asset
• Class 5 driver’s license and access to reliable vehicle while on shift
• Successful candidate will be required to provide 3 satisfactory references
• Successful candidates will be required to provide a current and satisfactory Criminal Record check with a Vulnerable sector search issued no later than six (6) months preceding the start date.
