Job description

WHO WE ARE
ONE KIDS PLACE Childrens Treatment Centre provides rehabilitation and related support services for children and youth (up to the age of 19) and their families living in the Districts of Muskoka Nipissing and Parry Sound. All staff will provide service within an interdisciplinary team environment committed to family-centered practice.

POSITION SUMMARY: Reporting to the Client Service Manager for Coordinated Service Planning the Service Facilitator is a key member of the Coordinated Service Planning team. The Service Facilitator provides family-centered strengths-based coordination of services for children and youth with multiple and/or complex special needs.

Serving as the single point of contact the SF works collaboratively across sectors to:

  • Develop implement and monitor a Coordinated Service Plan
  • Assist families in identifying priorities and setting meaningful goals
  • Ensure families are connected early and effectively with needed supports enabling positive outcomes and seamless transitions across home school and community settings.

JOB DUTIES:

Service Coordination & Family Engagement

  • Serve as the central point of contact for families and lead the development of a Coordinated Service Plan tailored to family goals and priorities.
  • Gather informed consent for documentation and information sharing.
  • Identify child and family strengths needs and goals; regularly review and update plans accordingly.
  • Facilitate family and youth participation in strengths-based culturally appropriate planning.
  • Initiate and monitor the implementation of the Coordinated Service Plan in collaboration with families and community partners.
  • Provide families with information support education and advocacy to help navigate the system.
  • Participate in and support funding application processes related to family/child needs.

System Navigation & Team Coordination

  • Facilitate referrals and coordinates access to appropriate services based on needs.
  • Build and sustain relationships with diverse community partners to ensure seamless service delivery.
  • Lead and chair service plan meetings; promote shared accountability across service providers.
  • Engage with Francophone Indigenous newcomer and other cultural/linguistic communities to ensure inclusive practices.
  • Address system barriers and identify innovative service solutions.
  • Collaborate with local resolution mechanisms for complex cases.
  • Maintain up-to-date knowledge of available services supports and funding opportunities.

Transition Planning

  • Lead transition planning for youth aged 14 in collaboration with family and stakeholders.
  • Support development of integrated future-focused goals aligned with youth aspirations.

Documentation Reporting & Compliance

  • Ensure accurate and timely documentation in client records in line with service standards including but not limited to notes meetings service plans funding applications and reports.
  • Comply with policies procedures and standards outlined by the MCCSS Coordinated Service Planning Guidelines Network Advisory Council and agency guidelines.
  • Participate in data collection outcome tracking and reporting on key performance indicators.


Professionalism & Leadership

  • Provide education to colleagues and partners on Coordinated Service Planning processes.
  • Demonstrate a strong understanding of relevant legislation (e.g. Child and Family Services Act Human Rights Code PHIPA).
  • Promote a positive team culture continuous improvement and inclusive service delivery.
  • Exhibit effective leadership in team building facilitation coaching conflict resolution and mediation.


QUALIFICATIONS:

  • Successful completion of a Social Services Worker Diploma and/or a diploma or degree in a relevant social services discipline and a minimum 3 years work and/or equivalent experience with vulnerable or at-risk populations including special needs
  • Proven knowledge and understanding of service coordination and effective system-based interventions related to children youth with Complex Special Needs and their families obtained by training and/or extensive experience in various capacities with this population
  • Excellent knowledge and experience working with the child and youth service system including ministry funding mechanisms
  • Commitment to strength-based principles of child youth and family-centeredness
  • Demonstrated ability to build and maintain relationships with diverse stakeholders and coordinate services in order to best meet the needs of the children youth and their families.
  • Ability to maintain strong professional boundaries
  • Excellent planning organizational presentation and reporting skills
  • Resourceful and independent with the ability to bring leadership within a team-oriented environment
  • Knowledge of child development
  • Flexible and able to adjust to changing priorities
  • Excellent communication (oral and written including public speaking) and interpersonal skills
  • Computer competency and adaptable in the use of computer software for the purpose of assessments documentation and communication.
  • Travel valid drivers license and proof of insurance required
  • Bilingual French & English is required

SOMMAIRE DU POSTE :
Rattaché(e) au/à la Responsable du service à la clientèle pour la planification coordonnée des services le/la facilitateur(trice) de services est un membre clé de léquipe de planification coordonnée des services. Il/Elle assure une coordination des services centrée sur la famille et axée sur les forces des enfants et des jeunes ayant des besoins spécifiques multiples et/ou complexes.

Interlocuteur unique le/la facilitateur(trice) de services travaille en collaboration avec tous les secteurs pour :


  • Élaborer mettre en œuvre et suivre un plan de services coordonné ;
  • Aider les familles à identifier leurs priorités et à se fixer des objectifs concrets ;
  • Sassurer que les familles sont rapidement et efficacement mises en relation avec les soutiens nécessaires favorisant ainsi des résultats positifs et des transitions harmonieuses entre la maison lécole et la communauté.


FONCTIONS DU POSTE:


Coordination des services et engagement des familles

  • Servir de point de contact central pour les familles et piloter lélaboration dun plan de services coordonnés adapté aux objectifs et priorités des familles.
  • Recueillir le consentement éclairé pour la documentation et le partage dinformations.
  • Identifier les forces les besoins et les objectifs de lenfant et de la famille ; réviser et mettre à jour régulièrement les plans en conséquence.
  • Faciliter la participation des familles et des jeunes à une planification axée sur les forces et culturellement adaptée.
  • Initier et suivre la mise en œuvre du plan de services coordonnés en collaboration avec les familles et les partenaires communautaires.
  • Fournir aux familles des informations du soutien de léducation et des services de défense des droits pour les aider à sy retrouver dans le système.
  • Participer et soutenir les processus de demande de financement liés aux besoins des familles et des enfants.


Navigation au sein du système et coordination déquipe

  • Faciliter laiguillage et coordonner laccès aux services appropriés en fonction des besoins.
  • Établir et entretenir des relations avec divers partenaires communautaires afin dassurer une prestation de services harmonieuse.
  • Diriger et présider les réunions sur le plan de services ; promouvoir le partage des responsabilités entre les fournisseurs de services.
  • Collaborer avec les communautés francophones autochtones nouvellement arrivées et autres communautés culturelles et linguistiques afin de garantir des pratiques inclusives.
  • Éliminer les obstacles systémiques et trouver des solutions de services novatrices.
  • Collaborer avec les mécanismes locaux de résolution des cas complexes.
  • Se tenir au courant des services du soutien et des possibilités de financement disponibles.


Planification de la transition

  • Diriger la planification de la transition pour les jeunes de 14 ans et plus en collaboration avec la famille et les parties prenantes.
  • Soutenir lélaboration dobjectifs intégrés tournés vers lavenir et alignés sur les aspirations des jeunes.


Documentation rapports et conformité

  • Assurer une documentation précise et ponctuelle des dossiers clients conformément aux normes de service y compris mais sans sy limiter les notes de réunion les plans de services les demandes de financement et les rapports.
  • Respecter les politiques procédures et normes définies par les lignes directrices du MCCSS sur la planification coordonnée des services le Conseil consultatif du réseau et les directives de lorganisme.
  • Participer à la collecte de données au suivi des résultats et à la production de rapports sur les indicateurs clés de performance.


Professionnalisme et leadership

  • Former les collègues et les partenaires sur les processus de planification coordonnée des services.
  • Démontrer une solide compréhension de la législation pertinente (p. ex. Loi sur les services à lenfance et à la famille Code des droits de la personne LPRPS).
  • Promouvoir une culture déquipe positive lamélioration continue et une prestation de services inclusive.
  • Faire preuve dun leadership efficace en matière de consolidation déquipe danimation de coaching de résolution de conflits et de médiation.


QUALIFICATIONS :

  • Baccalauréat en sciences sociales de préférencediplôme en techniques de travail social requis
  • Expérience de travail avec des enfants/jeunes atteints de le TSAF et leurs familles
  • Connaissance du développement de lenfant
  • Solides compétences interpersonnelles
  • Expérience de travail avec des personnes vivant un traumatisme
  • Expérience et connaissance du concept comportemental
  • Engagement envers les principes fondés sur les forces centrés sur lenfant la jeunesse et la famille.
  • Expérience de travail avec le système de services à lenfance et à la jeunesse
  • Capacité démontrée à établir et entretenir des relations avec diverses parties prenantes
  • Capacité à maintenir des limites professionnelles fortes
  • Excellentes compétences en matière de planification dorganisation de présentation et de reporting
  • Débrouillard et indépendant avec la capacité dapporter du leadership dans un environnement axé sur léquipe
  • Flexible et capable de sadapter aux priorités changeantes
  • Excellentes compétences en communication (orale et écrite y compris la prise de parole en public) et en relations interpersonnelles
  • Déplacement permis de conduire valide et preuve dassurance requis
  • Le bilinguisme français et anglais estrequis
  • Preuve de vaccination contre la COVID comme lexige le secteur
