STATUS: Temporary Part Time
CLASSIFICATION: Social Worker Grade II (50300)
WORK AREA: Central City Lodge; PCN #
HOURS: 0930-1830 Wednesday - Saturday (0.9 FTE 64 biweekly hours)
DAYS OFF: Sunday Monday Tuesday
SALARY: Grid 12 / $44.85-56.02 hourly

JOB SUMMARY

Providence Living is a non-profit Catholic-sponsored health care organization focused on long-term care assisted living housing and other community services to support seniors and their family members. We are committed to re-imagining the experience of seniors especially those living with dementia and to creating communities and fostering a culture where seniors are empowered to make decisions that affect their quality of life.

Reporting to the Director of Care this position is the sole social worker at the facility supporting the interdisciplinary program/team in achieving program and resident goals. Duties include conducting social assessments providing counselling services providing Long Term Care community resources for residents contributes to the effectiveness and development of community resourcesservicesand researchmaintainsa variety of records and provides leadership to students as assigned.

All staff have a part to play in preventing adverse events and in promoting safety forresidentsand staff by being aware of and adhering to all policies and procedures including promptly reporting any risks or safety concerns relating toresidentor staff safety.

QUALIFICATIONS

Education Training and Experience

  • Masters Degree in Social Work.
  • Two(2) yearspost-Baccalaureateexperience preferably in an extended or intermediate care setting including experience in resident and family counselling.
  • Current full registration with the British Columbia College of Social Workers.

Skills and Abilities

  • Working knowledge of relevant legislation AGA (Adult Guardian Acts) MHA (Mental Health Act) SDM (Substitute Decision Maker) and other related acts within the scope of practice.
  • Broad knowledge of community and regional resources.
  • Ability to work independently with consultation and guidance available and to work within an inter-disciplinary team.
  • Demonstrated abilityto:
  • provide counseling applicable to families and seniors including crisis counselling and brief therapeutic interventions
  • gather relevant data and make a multi-dimensional assessment
  • conduct social assessments and make recommendations
  • draw out and honour information that will contribute to understanding the residents situation
  • communicate effectively both verbally and in writing withresidents familiesstaffandcommunity agencypersonnel
  • work with residents and their families across the life spectrum
  • Skilled in the use of equipment and in techniquesappropriate fortreatment.
  • Working levelproficiencyin the Windows environment including applications such as Word and Excel.
  • A commitment to continued professional growth.
  • Physical ability to perform the duties of the job.

DUTIES

  1. Conducts social work assessments by methods such as interviewing the resident and/or family prior tomove-in collecting social dataregardingthe residents family environment living situation and financial status;identifiesthe residents and familys coping abilities andanticipatedneeds for discharge.
  2. In collaboration with the Clinical Care teamfacilitatesthe transition into care by planningorganizingand coordinatingmove-ins ensuring new residents and their families are supported throughout this process.
  3. Attends clinical/case conferences as a member of an interdisciplinaryteamfor clientreviewsadmissionsanddischargesbymethodssuchasprovidinginputfromasocialworkperspectiveonavarietyofcareissuesrelatedtotheresidentandensuringproper and effective treatment of lead case conferences asrequired.
  4. Counselsresidents and families to aidin thetransition to livingat Central City Lodge. Providesindividualandfamilycounsellingservicesbyexploringtheemotional response toidentifiedproblems personalconcernsand treatmentobjectivestofacilitatechangeincopingstylebehaviourattitudefeelingsandadaptiveresponses to an adverse life event.
  5. Engages in program/practice evaluation by methods such asidentifyingopportunities for change assessing available resources analyzing outcomes andidentifyingareas for improved efficiency and effectiveness in service delivery to enhance serviceto residentswithin designated programs and reduce gaps in services in the community.
  6. Prepares and maintains records by methods such as documentingresidentassessmentsinterventionsevaluationsmodificationsandgoalschartingresidentinformation preparing progress notes and reports in accordance with established standards policies and procedures to meet regulatory requirements and provide evidenceandrationalethatsupportfindingsandinterventionplan.
  7. Provides consultative and direct services toresidents families team members and communityhealthprofessionalsregardingcommunityresourcesandaccesstotheseresourcesbymethodssuchasidentifyingapplicableresourcesprovidinginformation onresourcesandsuggestingcontactsinordertofacilitateoptimalplanning.
  8. Maintains a variety of recordsrecordinginformationintoapproved formats collating data analyzing trends accounting forvariancesand recommendingactionplansasrequired.
  9. Attendsand/orleadsmeetingsin-servicesandotherinitiativespresenting relevant materials/topics to remain informed of departmental/practice activitiesandtocontributetoorganizationalinitiatives.
  10. Participates inprogram evaluation/quality improvement byidentifyingopportunities forchangeandformulatingrecommendationstobettersocialworkservices.
  11. Provides leadership to social work students as assigned by providing guidance and the opportunity toobserveand take an active role in the treatment of social workclients.
  12. Performs other related duties asrequired.
