Job description
Position Summary
Are you a LifeSkills Support Worker looking to highlight your knowledge, skills, and abilities in a new and dynamic environment? We currently have an opportunity for you to bring your excellent work ethic to our Primary Care department.

In accordance with the established mission, vision, values, and strategic priorities of Northern Health, and under the general supervision of Manager or designate, the Primary Care Life Skills Support Worker functions as a member of the interprofessional team, under the direction of the Team Leader. This position applies best practice to provide people with services as outlined in an individual’s plan of care, with the goal of maximizing independent living.

Primary Care Networks are partnerships between the Divisions of Family Practice representing BC’s family physicians, the regional health authorities, the First Nations Health Authority (FNHA) and local community partners, along with other community providers such as nurse practitioners. All partners are working together in their local communities and collectively to redesign .’s primary and community care system so that it works better for patients and health-care providers.

Qualfications
•Diploma from a recognized Social Services Support Worker educational program, or Residential Care Aide program, two (2) years’ recent, related experience working with people with complex health issues, or an equivalent combination of education, training and experience
• Or Grade 12 plus a Community Mental Health Worker Certificate and Addictions Studies Certificate plus three (3) years recent related experience working with adults with mental illness and/or addictions diagnosis; or an equivalent combination of education, training and experience.
• Valid . driver's license.
• Valid First Aid Certificate, CPR and Food Safe.

• Ability to analyze and resolve problems.
• Conflict resolution and crisis intervention skills.
• Advocacy skills.
• Basic computer skills including windows, word, internet and e-mail
• Ability to teach and work effectively in both groups setting and one-to-one.
